Appendix I. Student internship Contract Agreement (CMO 104, s.

2017)
INTERNSHIP CONTRACT AGREEMENT

Know all men by these presents:

This agreement made and entered this 6th day of February, 2019 at Abuyog, Leyte,
Philippines by and between:
Mr. Jude B. Asa, 24 years old, single, and a resident of Brgy, Caraye Javier, Leyte,
hereinafter referred to as a STUDENT.

AND
SKYGO MARKETING CORPORATION, a Corporation duly organized and existing
under the laws of the Republic of the Philippines, with the principal place of the business at
Brgy, Nalibunan Abuyog, Leyte, represented in this particular Act and Deed by Rene C. Serina,
Area Manager, hereinafter referred to as the CORPORATION.

ABUYOG COMMUNITY COLLEGE, an educational institution duly organized under


existing laws of the Republic of the Philippines and with license as a tertiary school granted by
the Commission on Higher Education, with principal place business at Brgy. Guintagbucan,
Abuyog, Leyte represented in this particular Act and Deed by. Mrs. Asuncion M. Villote,
College President – Designate, herein referred to as the SCHOOL.

DURATION OF INTERNSHIP
This agreement shall be for a period of TWO MONTHS (2) and two weeks (2) only
commencing on _February 6_, 2019 and terminating upon completion of the 400 Internship
Training Hours. The OFFICE shall have the option, without hearing to terminate such training
anytime when the best interest of the OFFICE so demands.
In other cases, modification and termination of this contract can only be done by the mere
agreement of the parties. Therefore, neither the common law requirement of the need for
consideration in order for the contract to be modified. If the contract has failed to be fulfilled, all
involved parties can annul the contract immediately. Termination of the contract has to be in
writing to the other parties. The resigning party equally has to inform the internship office in
writing.

ROLE EXPECTATIONS AND RESPONSIBILITIES


Student
1. The intern should complete a learning contract that defines the nature of the work
experience, the learning objectives, and academic expectations. Though this contract is
developed collaboratively with the faculty coordinator and the work supervisor, the
student needs to reflectively plan for the internship since it represents another aspect of
his/her academic program. It should reflect a level of academic rigor similar to that
encountered in the classroom.
2. The student must complete all of the work set down in the original contract. This
includes a complete evaluation of the internship that will be filed in the Provost’s office.
Only when this report is complete will credit be given for the internship.

Faculty Coordinator
1. The faculty coordinator serves as a mentor and facilitator to the student during the
internship experience. The coordinator should meet consistently with the student at
arranged times throughout the internship period to evaluate progress and give advice
when needed.

2. The faculty coordinator helps the student develop a learning contract or a suitable
alternative that is relevant and workable. The student should play a major role in the
initial planning and formulating stages of the internship in order to shape the learning
goals to meet his or her individual needs and to fully understand the extent of his or her
responsibility for the success of the experience.

3. The faculty coordinator should make several contacts with the work supervisor during the
internship. It is important to monitor the progress of the internship from a variety of
viewpoints and to maintain close relationships with all participants.

4. The faculty coordinator is responsible for seeing that all paperwork is completed by the
student prior to submitting a grade for the internship. Individual departments may
determine whether internships are given a letter grade or recorded pass/fail. The faculty
coordinator is responsible for the submitting required paperwork to the Provost’s Office
at the end of the internship.

5. No faculty member is obligated to take on the role of faculty coordinator or to find


internship sites for students. The internship should be regarded as a student initiative
with the faculty coordinator taking a supervisory role.
Work Site Supervisor
1. The site supervisor must assist with and agree to the terms established in the learning
contract.

2. The supervisor should take an active role in mentoring and guiding the student to help
meet learning goals and be available for meetings or conversations with both the student
and the faculty coordinator.

3. The site supervisor should complete the evaluation forms in a timely fashion and return
them to the faculty coordinator.

PLACE OF ASSIGNMENT
An internship, as defined by the Entrepreneurship Student Internship Program, is a
structured academic opportunity that allows students to apply academic skills and knowledge in
the work place. The workplace assignment of ___Jude B. Asa__ (name of student), is in Skygo
(business name) with the some of the expected tasks in recording, journalizing and preparation of
accounts pertaining to the operation of the said business.
The company is obliged to instruct the intern about their specific task or assignments,
rules and regulations of the company/business, including safety regulations, work and
communication routines. The intern is obliged to follow these rules and regulations applying to
the rest of the employees.

UNIFORM
The students are the one to furnish their own uniform prescribed by the Entrepreneurship
Student Intern Coordinator for four (4) days and an allowable one (1) day wash day.

WORKING HOURS
The internship is for a minimum of 400 hours. Commonly, a student can have at least 8
hours a day Monday to Friday which is equivalent to 48 hours a week, but, depending on the
company’s days of operation, a student can have an overtime of an additional one (1) day or 56
hours a week which will include Saturdays.

DAYS OFF
The student interns are allowed to have at least one (1) or two (2) days day off –
Saturdays and Sundays.

CONFIDENTIALITY
All confidential information, unless otherwise specified in writing shall remain the sole
and exclusive property of the OFFICE and shall be used by the STUDENT only for the
purpose intended, except as may be required by applicable law or legal process. No
stipulation contemplated hereunder shall be issued or published, circulated or cause or
permitted to be issued, published or circulated, by the STUDENT without prior written
consent of the company. Ref. Memorandum of Agreement.
