CONTRACT OF SERVICE

WHEREAS, SUNSHINE, WORLD COLLEGE OF TECHNOLOGY INC. is a

Technical and Vocational Education and Training Institution registered and recognized
accordance with Philippine laws, with business and postal address at Duterte St., Sta. Lucia
District, Pagadian City Zamboanga Del Sur represented by its School President- REBECCA
M. OLONGAYO;
WHEREAS, SUNSHINE WORLD COLLEGE OF TECHNOLOGY INC. is
offering
COMPUTER SYSTEMS SERVICING NC II a duly TVET registered program; and,

WHEREAS, it is the policy of SUNSHINE WORLD COLLEGE OF

TECHNOLOGY INC. to hire highly qualified trainer who support the TVET program
In pursuit of such high quality education standard as required by DEPED Program.

This contract is made on this 28th day of July 2023 at Pagadian City, Zamboanga del
Sur,
Philippines.

1. The INSTITUTION will hire FREDELYN G. CASTULO in accordance with terms

and conditions of this contract will start on of this contract for a period of (4) year.
The term of this contract will start on September 26, 2019 until September 26, 2023.

2. The trainer agrees to serve the INSTITUTION for the number of days in the official
school and to perform other duties assigned by the school president; and to work
cooperatively with the staff and administration of the institution.

3. The TRAINER will perform the following specific duties and responsibilities to wit;

a. Conduct orientation to trainees on school rules and regulations;

b. Check daily attendance of trainees;
c. Motivate trainees to complete the requirement for training;
d. Produce Competency- Based Learning Materials to Trainees;
e. Produce trainees Performance report;
f. Update progress chart;
g. Implement equipment and facilities maintenance schedule;
h. Develop learning materials;
i. Conduct Training Evaluation and recommends improvements;
j. Perform other function as authorized by the school president.

4. The TRAINER will be suspended or discharged for good cause shall be determined
in the exclusive discretion of the Board of directors. It is specifically understood that
good cause for discharge shall include but not limited to: inadequacy of teaching,
misconduct,
neglect of duty, physical or mental incapacity, actions involving moral turpitude,
violation of the terms of this agreement or SUNSHINE WORLD COLLEGE OF
TECHNOLOGY INC. policy, or any conduct not in keeping with the conduct
 tending to reflect discredit upon the institution or tending to impair the TRAINER’S
usefulness in her capacity as trainer.

5. The INSTITUTION hires the TRAINER at a salary of Php 30,000.00. Per month.

6. The TRAINER is responsible to pay SSS AND PHILHEALTH.

7. The TRAINER has not been subject to any administrative cases nor has been
previously dismissed from the service due to any offense.

8. It is agreed that the condition of this contract shall only be changed by mutual written
agreement of the TRAINER and INSTITUTION. This is the sole agreement
between the parties and no other representatives, be they oral or written, are binding
between the parties.

1. The INSTITUTION will hire RICARDO H. OLONGAYO in accordance with

terms and conditions of this contract will start on of this contract for a period of (3)
year. The term of this contract will start on February 03, 20219 until October 30,
2023.

2. The trainer agrees to serve the INSTITUTION for the number of days in the official
school and to perform other duties assigned by the school president; and to work
cooperatively with the staff and administration of the institution.

3. The GUIDANCE COUNCILOR will perform the following specific duties and
responsibilities to wit;

a. Conduct orientation to trainees on school rules and regulations;

b. Check daily attendance of trainees;
c. Motivate trainees to complete the requirement for training;
d. Produce Competency- Based Learning Materials to Trainees;
e. Produce trainees Performance report;

nor has been previously dismissed from the service due to any offense.

8. It is agreed that the condition of this contract shall only be changed by mutual written
agreement of the GUIDANCE COUNCILOR and INSTITUTION. This is the sole
agreement between the parties and no other representatives, be they oral or written,
are binding between the parties.

2. The trainer agrees to serve the INSTITUTION for the number of days in the official
school and to perform other duties assigned by the school president; and to work
cooperatively with the staff and administration of the institution.

3. The TEACHER will perform the following specific duties and responsibilities to wit;

a. Conduct orientation to trainees on school rules and regulations;
