AFFIDAVIT OF ADMISSION OF PATERNITY

I, RITO J. ESTALANI, JR., after having been duly sworn to in accordance with law
hereby depose and say:

1. That I am single, of legal age, Filipino citizen, and a resident of A. Mabini

St., Macrohon, Southern Leyte, Philippines;

2. That sometime in October 2011, I met Mary Rose T. Bantug of legal age,
Filipino citizen and a resident of Brgy. Ibarra, Maasin City, Southern Leyte,
with whom I had a relationship;

3. That as a result of said relationship, she conceived and gave birth to a

child on August 15, 2017 and we named the child, RAEL MATTHIEU;

4. That I am the biological father of the child, RAEL MATTHIEU;

5. That at the time of the birth of said child, I am not legally married to Ms.
Mary Rose T. Bantug, her biological mother;

6. That I hereby admit under oath my paternity of the child, RAEL

MATTHIEU;

7. That I am executing this affidavit to attest to the truth of the foregoing

facts and for whatever legal purposes it may serve.

IN WITNESS WHEREOF, I have hereunto affixed my signature this 17th day of

August 2017 at Maasin City, Philippines.
