CLINICAL POSTING OBJECTIVES

 Appreciate the history and development in the field of paediatric and paediatric nursing as a

 To provide round the clock state of the art diagnostic services/facilities in clinical pathology,
including blood bank, clinical biochemistry, microbiology and radiology, including
ultrasound and perinatal screening test.

DESCRIPTION OF BAI JERBAI WADIA HOSPITAL FOR CHILDREN

 Before the onset of paediatrics as an independent discipline, childcare was limited to using
adult medication on to children, who were seen merely as mini version of adults. In 1929, Sir
Ness Wadia and Sir Cusrow Wadia built the Bai Jerbai Wadia hospital for children, in
memory of their mother Bai Jerbai Wadia.
 Emerging as India’s first specialized paediatrics hospital, it was dedicated exclusively to
health care for children.
 Located in the heart of the city, the hospital pledges to extend its services to people,
regardless of their socio- economic status, believing strongly in the fact that quality healthcare
should not be restricted to only certain sections of society.
 The building is a Grade 2 heritage structure located at Acharya Donde. Marg. Parel, Mumbai.
It comprises ground plus two upper floors and has a built – up area of 1,40,000 Square feet
which consists of 5 blocks (A,B,C, D& E).

HISTORY OF BI JERBAI WADIA HOSPITAL FOR CHILDREN:

 Bai Jerbai Wadia Hospital symbolizes the philanthropic tradition of the Wadia family. The
foundation stone of the hospital erected in the memory of Sir Nowrosjee Wadia by his two
sons, the late Sir Cusrow N Wadia and the late Sir Ness N Wadia, in a joint venture with the
State Government and the then Bombay Municipality, was laid on 9th June, 1925.
 The 120 bedded hospital was designed by Architect George Wittet, who was also the designer
of the Gateway of India. NWMH was declared open on 13th December, 1926, the 27th death
anniversary of Sir Nowrosjee Wadia by the then Governor of Bombay, Mr. H.E. Leslie
Wilson. Dr. M.V. Mehta was the first Hon. Principal Medical Officer of the hospital.
 Paediatric’s made a beginning in Mumbai in 1928, when Dr. George Coelho, rightly called
Father of Indian Paediatrics, became the superintendent of the BJ Hospital for Children-the
first hospital in India.
 He remained the head of the Department of Paediatrics till 1953.
 In 1929, Bai Jerbai Wadia Hospital for Children become a separate independent children’s
hospital.
 The hospital consists of 412 beds with 38 private rooms and 5 general ward with OT area,
CSSD, HEMAT, NICU& IPCU, CATH LAB etc.

Mission is to carry forward the philanthropic legacy of the Wadia family and deliver world -class
treatment in paediatrics care at affordable cost.

Vision is to provide top of the line affordable healthcare services at par with international standards
through preventive, curative, intensive and rehabilitative methods as well as spreading awareness
about the same through imparting quality health education. It is also endeavour, as a teaching
institute, to provide quality education to aspiring doctors.

 Total budget of the hospital is not disclosed to us but it has been told that 50% is given by the
Government and 50% by the BMC (Bombay Municipal Co-operation) .

RECRUITMENT PROCESS AND SALARY

• Employees are recruited yearly according to the need of the input of the patients in the
hospital

• Salaries of the employees are given per month.

• Permanent staffs were paid Rs 45, 000 per month while the employees on contract were paid
Rs 20,000.

• Different increments and allowances are given to the employees according to the policies and
protocols set by the hospital.

STAFF WELFARE ACTIVITIES:

Continuing Education program: Staffs are encouraged and sent for various workshops, seminars, etc.
which are conducted by different organizations and institutions.

HOSTEL FACILITIES

 Nursing hostel is attached to the hospital.


 Hostel is divided into 3 floors and each floor contains 30 rooms.
 Dining area and pantry are accommodated on ground floor.

Mean Timings:

 Breakfast: 6:45am to 8:15 am


 Tea time: 9:30 to 9:45am
 Lunch: 12pm to 2:00 pm
 Single room is provided for senior nurses in nursing hostel.
 2 to 3 junior nurses sharing a single room in nursing hostel.
 3000 Rs. for the residential and food is charged from every nurse.

MEDICAL FACILITIES
All employees of get 50% off for their medical checkup and treatment. No medical facilities is
provide for their family members.

LEAVE FACILITIES

 Earned Leave:- 30 days per year


 Casual leave:- 10 days per year
 Medical leave:- 10 days with full pay or 20 days with half pay per day.
 Festival holidays:- 12 days per year.
 Abortion Leave:- 42 days.
 Maternity leave:- 135 days (given for only 2 children)
 After every night duty staff nurse is given 1 day off.

 Morning Shift :- 7:00 am to 3 pm


 Evening Shift :- 3pm to 10 pm
 Night :- 10 pm to 7:00 am 

On Saturday:

 Duty Timings For Ward Incharges - 7:00 am - 12:00 pm 

For N.S. & A.N.S. :- 9:00 am to 5:00 pm

For N.S. and A.N.S., there are no night shifts.

During evening shifts, there is one nursing supervisor for overall supervision of the hospital.

THE STAFF NURSES WORK ON ALL SHIFTS

 Casualty Medical Officer and house surgeons are available 24 hours all days.
 Specialist Doctor's are available round the clock in Major specialties like Surgery and
Pediatrics.
 Duty Doctor is available on call in Anesthesia, Pathology, Radiology.
 The decision to call a specialist is that of the treating doctor.
 Failure to respond to a call is regularly monitored by the Medical Superintendent of the
hospital.
 Emergency cases are attended to promptly.
 In serious cases, treatment / management gets priority over paper work like registration and
medico legal requirements. The decision rests with the treating doctor.
 Emergency Operation Theatre is maintained on a regular basis to ensure that it is usable at all
times.
 Free telephone is available at the entrance of Casualty for patients and their attendants.
 Ambulance services for bringing and referred patients are available.
 Tie up with K.E.M. if patient has to be referred.
 Trauma patients or any other persons can bring the victim to the hospital Casualty/ labour
room in case of pregnant women.
 Patient is attended promptly by the emergency medical officer / gynecologist in Labour Room
if pregnant.
 If required she seeks the consultation of specialist available round the clock.
 The patient is discharged if found fit after giving the treatment or is admitted if serious.
 The patient is assessed and police is informed in case it is of Medico Legal cases.
 Police Post is exists in campus itself.

SERVICES AT BAI JERBAI WADIA HOSPITAL FOR CHILDREN

The hospital served as a community hospital for the last 80 years and has daily medical and surgical
OPD to take care of common day to day health care needs of children. Well baby clinic are run thrice
a week for providing free immunization services and dietary advice. Three large medical wards, two
surgical wards, an orthopaedic ward, NICU and a PICU meet the needs of sicks children who requires
hospitalization.

 Students from different colleges are brought together for training.


 Immunization schedule is followed in Immunization room, Postnatal ward & Neonatal ward
as well.
 Special concession provided to the patients with yellow card holder in sort of cost of
treatment.
 Poor mothers whose babies are admitted in the NICU 50% concession provided for the
treatment cost.
 For handicapped patients 100% concession provided for the treatment cost i.e., treatment
provided is free of cost.
 If female child born, then after 5 years BMC Government provide some facilities and services
to the girl child.

GROUND FLOOR

 RECEPTION:-

This department is open 24 hours for any kind of enquiry, direction, guidance to the clients and
their relatives . Two receptionist hold the reception round the clock in shift(morning and evening) for
providing hospital leaflets, admission cards and registration for new admission number.

OUT PATIENT DEPARTMENTS (OPD)

 Timing : (From 8.00 a.m. to 4.00 p.m.)


 Registration timing 8.00 a.m. to 1.00 p.m.
 Every outpatient seeking treatment at the hospital is registered and issued a Card for
recording symptoms, diagnosis and treatment being provided.
 Boards indicating units on duty on various days are displayed outside each OPD.

PHYSIOTHERAPY AND OCCUPATIONAL THERAPY UNIT

 Physiotherapists entering this field of pediatrics will need to be prepared to adapt to the
techniques that they have learned in relationship to adults to the ever-changing needs of the
growing child.
 The physiotherapist will inevitably work within the context of the child and his family and
have opportunity to work with the child in a range of situations extending into home, pre-
school groups, education and leisure activities. Because of the complex needs of the child and
the family, the physiotherapist may work with many other disciplines including medical,
nursing, social work, educational and care staff, psychological and psychiatric teams as well
as speech and occupational therapists.

CLUB FOOT CLINIC

 In July 2013, Bai Jerbai Hospital for Children joined hands with the Miracle Feet Foundation
and CURE International India to introduce a Clubfoot Clinic at the hospital.
 The main aim of this clinic is to treat clubfoot deformities in new born children so as to
prevent further disabilities during the course of their lives.
 The hospital acts as a nodal centre for the treatment of this disease and offers affordable
healthcare to every child who needs it.

NUTRITIONAL REHABILIATION CENTRE

 1st October 2019, Mumbai:  Bai Jerbai Wadia Hospital for Children (BJWHC), India’s first
and largest Pediatric hospital in association with UNICEF formally inaugurated the Nutrition
Rehabilitation Centre facility at BJWHC.
 The state-of-the-art Nutrition Rehabilitation Centre will address child survival and
development with equity among the most deprived urban poor communities in and around
Mumbai by providing advanced treatment support with a focus on early childhood
development for children with Severe Acute Malnutrition at BJWHC.

 Bai Jerbai Wadia Hospital for Children has a separate kitchen catering to all the indoor
patient.

CATH LAB

 By understanding the importance of patients receiving from other parts of Maharashtra,


Wadia management has taken the noble step to come up with Outreach OPD centres. Heart
specialists from Bai Jerbai Wadia Hospital for Children would conduct OPD’s on alternate
Sundays, twice a month. This will make life easier for those children as most procedures and
follow-ups can be done at their doorstep in Nashik itself.”

MEDICAL WARD
 A hospital ward in which patients are being treated by drugs rather than surgery.
 1 nursing station, 1 store room &, 1 examination room, Separate wash room for patient and
staff.
 There are 3 medical wards in first floor i.e. dolphin ward, lion ward and tiger ward.
 There is 1 big hall in ward contains 43 beds and extra 7 beds are placed in the corridor.
 On duty 4 staff nurses in morning shift, 3 in evening and 3 in night shift.
 Bio medical waste is disposed in yellow, blue and black dustbins white dust been.
 Duty shift of medical ward staff is as same as other staff shifts.

SURGICAL WARD:

 A hospital ward in which patients are being treated by drugs after the surgery.
 1 nursing station, 1 store room &, 1 examination room, Separate wash room for patient and
staff.
 There are 3 surgical wards in first floor i.e. bear ward, camel ward and elephant ward.
 There is 1 big hall in ward contains 50 beds in bear ward and 35 beds in elephant ward and
camel ward or 7 extra beds are placed in the corridor.
 On duty 4 staff nurses in morning shift, 3 in evening and 3 in night shift.
 Bio medical waste is disposed in yellow, blue and black dustbins white dust been.
 Duty shift of medical ward staff is as same as other staff shifts.

NICU:-

 NICU is intensive care unit which provide specializing care to ill or premature new-born.
NICU of BAI JERBAI WADIAHOSPITAL FOR CHILDREN know as one of the largest
NICU in India it is about 85 bedded and the hospital plan to add more beds, taking the counts
to 150 by next April.
 The hospital has also started the processes to become a dedicated organ transplant centre for
children and it is also binds with various NGOs and foundations.
 Cuddles foundation one of the important foundation that is working Wadia Hospital. It
provides holistic and nutritional support to 35,000 underprivileged children fighting cancer
across India.

NICU of BAI JERBAI WADIA HOSPTIAL divided into further 6 sections that is:

 section A(10 bedded)-(isolated patient)


 section B(10 bedded)-(surgical patient)
 section C(20 bedded)- (septic patient)
 section D(20 bedded)- (clean baby)
 section E(15 bedded) -(Grover and feeder)
 section F( 10 bedded)- (KMC giver)

Nurses ratio:

 The staff nurse must have minimum of 3-year work experience


 Nurse patient ratio of 1:2 maintained throughout day and night.
 Additional Supervisor nurse who is the overall in charge.

STAFFING PATTERN:

 One nursing room and one changing room.

 In NICU there were 90 radiant warmer & 40 ventilators for sick neonates.

 On duty 12 staff nurses in morning shift,10 in evening and 10 in night shift.

 2 ANM staff in every shift

PEDIATRIC INTENSIVE CARE UNIT :-

 The Pediatric Intensive Care Unit (PICU) is a multidisciplinary unit that provides care for
infants, children and adolescents who become critically ill or injured.
 PICU is intensive care unit which provide specializing care to ill where critically ill children
are admitted. PICU of BAI JERBAI WADIA HOSPITAL FOR CHILDREN are 16 bedded
where 2 bed are for isolation children. The hospital has also started the processes to become a
dedicated organ transplant centre for children and it is also binding with various NGOs and
foundations.

AUDIOLOGY AND SPEECH THERAPY

 Audiologists employ various diagnostic tests to determine whether someone can hear within
the normal range, and if not, which portions of hearing (high, middle, or low frequencies) are
affected and to what degree.
 The department offers management and treatment of hearing loss and speech/language
disorders to children. The department collaborates with the Paediatric ENT specialists to offer
the most efficient care to each and every child.

 Paediatric Speech Therapists work with children with speech, language, communication or
swallowing difficulties.

CONCLUSION

Bai Jerbai Wadia hospital for Children is one of the best institutions for pediatric services in the
country. It is famous in India for its pediatric services and research and training facilities for students
under these specialities. All the members of the hospital were co-operative and I gained a great
knowledge by this clinical visit.
