Bishop Enrique V. Macaraeg certifies that:

1. He is a duly appointed Roman Catholic Bishop who desires to become a


Corporation Sole representing the Diocese of the Roman Catholic
Church comprising the whole civil province of Tarlac (except Camp
Servillano Aquino in San Miguel, Tarlac City, which belongs to the
jurisdiction of the Military Ordinariate of the Philippines) in the
Philippines;

2. The rules, regulations and discipline of the Roman Catholic church are
consistent with becoming a corporation sole and do not forbid it;

3. The purpose of this nonprofit corporation shall be religious worship as


well as the dissemination of the teachings of catholic church and to
organize and establish local parishes within the province of Tarlac;

4. That in case a vacancy occurs in the position of Bishop of Diocese of


Tarlac, the successor shall be chosen by the Roman Catholic Pope in
accordance with the rules, regulations and discipline of the Roman
Catholic church.

5. He is charged with the administration of the temporalities and


management of the affairs, estate and properties of the Roman Catholic
