AFFIDAVIT OF CONSENT TO TRAVEL ABROAD

I, CHRISTOPHER TUMLOS COS, of legal age, Filipino

citizen, married and a resident of Imus, Cavite, Philippines but
presently on overseas work as Computer Programmer at Tabuk,
Riyadh, Saudi Arabia, under oath, depose and state that:

I and my wife, Luz Llanaresas Cos, were blessed with two

(2) children, Syra Gail L. Cos, of legal age and Kyle Dwight
Llenaresas Cos, 14 years old, a high school student at Perpetual
University, Las Piñas City.

That our children will be travelling to Singapore as tourists

together with their cousins, uncle and grandparents, Atty. Allan
B. Cos and Jocelyn T. Cos, scheduled on December 17, 2014 to
January 2, 2015.

As such, I am giving my consent and approval voluntarily for

my minor child Kyle Dwight Llenaresas Cos, to travel to
Singapore in the company of my parents and his grandparents
Atty. Allan B. Cos and Jocelyn T. Cos.
