Assignment 3
Process Safety and HAZOP Study
(100 marks)

Perform HAZOP study for the process plant as a team using the latest P&ID. Each group is required
to do at least 2 nodes which include at least one reactor system and one separation system
(complex unit operator such as distillation columns should be prioritised if your group has multiple
separation units/systems). All group members must be present in the HAZOP workshop/discussion
and conduct brain-storming with the aid of guideword to identify potential deviation, cause,
consequence, safeguard, and recommendation. The focuses of this assignment are:
 Conduct HAZOP analysis to identify plausible process hazard, identify adequacy of process
safety and recommendation proposal.
 Improve process safety and control, and update the P&ID accordingly.

(A) HAZOP Study Report


1) Prepare a HAZOP study report (refer to the template provided) which should include at least the
following information:
 Executive summary
 Introduction
 Process description of each node
 Methodology and assumptions in carrying out the HAZOP study
 Study Team
 Analysis of main findings and actions arising from the study

2) Update the P&ID (as new revision, i.e. “After HAZOP P&ID”) based on the HAZOP findings and
recommendations.

(B) HAZOP Worksheet
Prepare HAZOP worksheet based on the template provided. Only one worksheet is needed for each
node (system). Each team member has to agree on all the discussion noted in the worksheet prior
submission. All recommendations provided in the worksheet must be translated into the “After
HAZOP P&ID (new revision)”. Student may take turn to be the scribe, chair, process engineering,
operation, etc to complete the HAZOP worksheet for different node. Node identification may refer to
the system as identified in Process and Plant Design I.

The report and worksheets will be evaluated based on (but not limited to) the requirements shown
in the template provided, the basic requirements as listed above and the overall presentation.

Submission Documents
Submit the following as four separate documents.
1) HAZOP study report with cover page.
2) HAZOP worksheets with cover page.
3) Process Drawings (A4 size) used for HAZOP study (before HAZOP) which shall include the
following with the correct revision number:
 Process Drawing Index
 Symbols, Legend and Abbreviations
 Process Flow Diagram (PFD)
 Material and Energy Balances (Stream Tables)
 Piping and Instrumentation Diagram (P&ID) with node mark-up
 Cause & Effect Matrix
4) Updated P&ID and Cause & Effect Matrix based on HAZOP recommendations (after HAZOP;
this should be a new revision drawing)
