Direction: Please complete all the required information asked for. In Part I. For Part II, you are to evaluate the Table of
Specification prepared by the group paired to you using the criteria listed in the checklist. Simply indicate whether the criterion
was COMPLIED WITH by putting the number 1 or NOT COMPLIED WITH by putting the number 0 on the appropriate cell. DO NOT
LEAVE ANY CRITERION UNMARKED. When all criteria had been rated, get the total by adding all the 1s and place the sum on the
cell calling for the TOTAL, and COUNTING ALL 0s and placing the TOTAL COUNT on the cell calling for this information.
