WHEREAS, the GREENDALE & GUADALUPE TRICYCLE OPERATORS AND DRIVERS ASSOCIATION
(GGTODA) is a legitimate organization duly registered with the Department of Labor and
Employment, with the registration number R08-NLFO-2019-03-WA-1632, located at Brgy. 105,
San Isidro Suhi, Tacloban City, Philippines.

WHEREAS, in the meeting of GGTODA held at Brg. 105 San Isidro Suhi, (GGTODA Outpost)
Tacloban City. It was agreed and unanimously decision by the board of Directors to have a BIR
certification.

RESOLVED AS IT IS HEREBY RESOLVED, to give Mr. Alfredo M. Bautista Jr. GGTODA Elected
Secretary, authority to represent and process any documents needed in order for us (The
Officers and Member) to have a BIR certification.
