H. Course Description:

This course deals with meta concepts of a person, health, environment and nursing as viewed by the different theorist; likewise,
it includes non-nursing theories such as system, development and change theories. It presents how the concepts and criteria serve
as guide to nursing practice. It further deals with health as multi-factorial phenomenon and the necessary core competencies that the
nurse needs to develop.

Hi! Future nurses, let us introduce ourselves to everyone starting with me your clinical instructor and class adviser. I’m Cheryl Peral
Porpayas-Alvarado 26 years old, graduated from this institution Surigao Education Center last March 2015. I took the Nursing Licensure
Exam last November 2015 and passed. I’ve worked at Miranda Family Hospital as a Staff Nurse year 2016 for about 4 months and after
that I worked at Surigao Medical Center the same year (2016) until 2017. Presently, I’m working here as clinical instructor, I started here
august 2017. And now I will call your name individually to introduce yourselves with the pattern of name, age, school graduated from and
your goals/dreams in life.
Now that were done introducing ourselves to every one, I will now introduce to you the overview of the course and it’s requirements.
In this course you will learn all about the foundations of nursing; it’s concept, theory, theorists, principle, purposes, paradigm and it’s origin.
The requirements are; you must have a book of Theoretical Foundations of Nursing and Student’s Clinical Manual which you can purchase
at the finance department, clear book color green for your compilations of quizzes, assignments, exams, and etc., when it comes to grades;
quizzes 30%, participation 15%, requirements/assignments 15%, exam 40% for a total of 100%. In order to pass in this course, you must
acquire an average of 75% at the end of term. For any queries just pm me at my messenger account or at my email
porpayasc@gmail.com.
For now, let me have an introduction of nursing to you.

Nursing
What is the essence of nursing today? Over the years, nursing is described as both an “art” and a “science”. the art of nursing practice
is grounded in scientific principles. Nursing theories provide the scientific and professional foundation to nursing knowledge development.
Moreover, nursing theories reinforce the nursing profession’s emphasis on evidence-based practice (EBP)-practice with meaning.
As the nursing profession continues to expand its own body of knowledge, likewise concepts and theories develop to support nursing’s
practice component. Development and clarification of nursing theories applied to the practice of professional nursing concretely describe
what nurses do, why and how. On the whole, nursing theories are essential to the evolution of nursing knowledge and professional practice.
Therefore, nurses should value the importance of nursing theories for growth, excellence, and professional survival. To appreciate
nursing theories, nurse graduates, nurse educators on nursing theories and their application. This is with the aim to mold future nurses who
will provide theory-driven practice towards excellent, individualized, quality, ethical, and humane patient care.

“The end or purpose of nursing is the well-being of other people…it is a moral end. That is, it involves the seeking of a good, and it
involves relationships with other human beings. The science learned and the technical skills developed are designed and shaped by this
moral end.” (Curtin and Flaherty, 1982,p.86).

1. Concept
 A mental idea of phenomenon
 A comprehensive idea or generalization
 An idea that brings diverse elements into a basic relationship
 A unit of knowledge abstracted from a set of characteristics attributed to a class of objects, relations, or entitles.
 A unit of thought
 A general idea formed in the mind
 Something understood or retained in the mind, from experience, reasoning, and/or imagination; generalization or abstraction of
a particular set of instances or occurrences.

2. Theory
 A set of statements that tentatively describe, explain, or predict relationships among concepts that have been systematically
selected and organized as an abstract representation of some phenomenon. These systematic organized perspective serves as
guides for nursing action in administration, education, research, and practice.
 Literally means “a vision” or a “beholding scene”
 in scientific terms, a theory implies that something has been proven and is generally accepted as being true
 An explanation of a set of interrelated concepts that gives a systematic view of phenomenon and that has explanatory and
predictive qualities based upon proven hypotheses and verified multiple times by detached groups of researchers.

3. Principle
 A law or rule that has to be, or usually is to be followed, or can be desirably followed, or is an inevitable consequence of

 Characteristics of a Theory
 Must be consistent with reality
 Theories can interrelate concepts in such a way as to create a different way of looking at a particular phenomenon/fact.
 Theories are logical in nature-sensible/reasonable
 It can be the bases for hypothesis that can be tested or expanded
 Should be relatively simple yet generalizable
 It contributes to and assists in increasing the general body of knowledge within the discipline through the research implemented
to validate them.
 It can be used by practitioners to guide and improve their practice.
 It must be consistent with other validated theories, laws, and principles but will leave open unanswered questions that need to
be investigated.

 Components of a Theory

1. Concepts
- A theory is composed of interrelated concepts.
- Concepts help to describe or label phenomena.

 Example: Using Levine’s Conservation Model in Nursing Practice, there are concepts that affect the nursing practice- the
“why’s of nursing actions.” The three major concepts that form the basis of the model and it’s assumptions are as follows:
(1) Conservation (2) Adaptation (3) Wholeness

 King’s Theory of Goal Attainment in Nursing Practice. According to this model, the concepts that are critical to goal
attainment in nursing practice are as follows:
(1) Personal Systems (2) Interpersonal Systems, and (3) Social Systems

2. Definitions
- The definition with the description of a theory convey the general meaning of the concepts in a manner that fits the theory.
These definitions also describe the activity necessary to measure the constructs, relationship, or variables within a theory.

 Example: Levine’s Conservation Model defines conservation as the keeping together of the life system; that individuals
continuously defend their wholeness. Accordingly, wholeness exists when interactions or constant adaptations to the
environment permit assurance of integrity. In addition, the Model defines adaptation as the ongoing process of change

whereby individuals retain their integrity within the realities of their environment.

3. Assumptions
- are statements that describe concepts or connect two concepts that are factual
- Are the “taken for granted” statements that determine the nature of the concepts, definitions, purpose, relationship, and
structure of the theory.

 The assumption in Levine’s Conservation Model are that: individuals continuously defend their wholeness; adaptation is an
ongoing process of change; nursing is human interaction; nurses promote wholeness through the use of conservation
principle.
 The assumptions in King’s Goal Attainment are that: human beings are the focus of nursing; the goals of nursing are or the
injured; and care of the dying.

4. Phenomenon
- Is an aspect of reality that can be consciously sensed or experienced. Nursing theories focus on the phenomena of nursing
and nursing care.

 Example of phenomena in nursing include caring, self-care, and client responses to stress.

 In Levine’s Conservation Model, phenomena include: person, nursing, health and environment. The King’s Goal Attainment
Theory phenomena include: social systems, health, perception, and interpersonal relationships.

 PURPOSES OF NURSING THEORY


 In Education
- Nursing theory was used primarily to establish the place of nursing profession in the university
- It is more established in the academia rather than the clinical practice.
- In 1970’s and 80’s, many nursing programs identified the major concepts in one or two nursing models, organized these concepts
into a conceptual framework, and attempted to organize the curriculum around the framework

 In Research
- Nursing Research identifies the philosophical assumptions or theoretical framework from which is proceeds.
- Is more informed by midlevel theories- Focus on the exploration of concepts such as pain, self-esteem, and learning.

 In Nursing
- Critical Theory (describe theories that help answer how social structures after a wide variety of human experiences from art to
social practices) is used to explain how social structures (e.g. race, gender, sexual orientation, economic class) affect patient
experiences and health outcomes)

 In Clinical Practice
- Primary contribution in the clinical setting is the facilitation of reflection, questioning, and thinking about what nurses do in the
clinical area.

Hi! Future nurses, let’s tackle today the nursing paradigm (open forum- VC)

 Nursing Paradigm
 A model or paradigm that explains the linkages of science, philosophy, and theory that is accepted and applied by the discipline.
 The elements of nursing paradigms direct the activity of nursing profession, including knowledge development, philosophy, theory,
educational experience, research, practice, and literature identified with the profession.
 Nursing identified its domain in a paradigm that includes four linkages; the person, health, environment/situation, and nursing

 Four Major Concepts


 Person/Client- recipient of nursing care (includes individuals, family, groups, and communities)
 Environment- internal and external surrounding that affect client; also includes people in the external environmental
 Health- the degree or wellness or well being that the client experiences
 Nursing- the attributes, characteristics, and actions of the nurse providing care on behalf of, or in conjunction with, the client

 Importance of Nursing Theories


 Aim to describe, predict and explain the phenomenon of nursing
 Provide the foundations of nursing practice, help to generate further knowledge and indicate in which direction nursing
should develop in the future
 Help distinguish what should form the basis of practice by explicitly describing nursing.
 Help provide better patient care, enhanced professional status for nurse, improved communication between nurses, and
guidance for research and education
 The main exponent of nursing- caring-cannot be measured. It is vital to have the theory to analyze and explain what nurses
do.
 Establish a unique body of knowledge

 Importance of Theoretical Frameworks


- Foundation of any profession is the development of a specialized body of knowledge

- Theories should be develop in nursing, not borrow theories form other disciplines
