    Please note that this activity is equivalent to a Quiz

    1. Make a Collage depicting your own spiritual development. Apply your own concept and
    meaning of spirituality and quality of later life. You may use any materials available to you in
    making your collage.

    2. Write a 3-paragraph reflection paper on your own Life experience regarding the care of
    terminally-ill relatives or elderly family members who were near the end of life as well as your
    own experience of Death and Bereavement in the family.

    Focus Questions:
    What were the religious practices you observed while caring for your relative/elderly family
    member?
    Were the spiritual needs of your relative/elderly family member addressed?
    What methods of spiritual care did you or the caregiver use?
    Was the quality of life achieved in the aspect of spiritual belief of your relative/elderly family
    member?
    How did you and the members of your family or relative overcome the loss of your loved one?
