PETITION
Petitioner unto this Honorable Court, most respectfully states:

1. That he is a citizen of the Philippines, thirty four(34) years of age,


married and has been a resident of the country since birth;

2. That he was born on 28 September 1985 and is presently


residing at Unit 110, Lorenz Executive Apartelle, Ibapo, Mactan,
Lapu-Lapu City, Cebu, Philippines, with residence telephone
number (032) 493-5183 and current office address at Cortes-
Rizon Law and Accounting Offices, 2F, Sebastien Hotel, Ibapo,
Mactan, Lapu-Lapu City, Cebu with telephone number (032) 495-
1077 and in which City and jurisdiction he was given a Notarial
Commission under Number 461-L, and thereafter re-appointed as
Notary Public for the term starting January 16, 2018 up to
December 31, 2019.

Copy of Petitioner’s Notarial Commission order of appointment is


attached herewith as Annex “A”

3. That he passed the Bar Examinations given in the year 2016,


under the Roll of Attorney No. 67543, and he has been admitted to
the practice law in the Philippines on May 24, 2017, with current
Professional Tax Receipt number 7402815 dated January 9, 2019
and Lifetime Integrated Bar of the Philippines (IBP) membership
No. 005620 dated January 10, 2017;
Copy of the Certificate of Membership in the Philippine Bar issued
by the Supreme Court of the Philippines and receipts of Privilege
Tax and Membership Dues are attached herewith as Annexes “B”,
“C” and “D”, respectively.

4. That in case of any change of address, he will inform the Executive


Judge within ten (10) days of any change in the previously given
address;

5. That his commission as Notary Public will end on the 31st of


December 2019. He desires to renew his commission as Notary
Public for the City of Lapu-Lapu and he possesses all the
qualifications and none of the disqualifications for said office;

6. That he has no other application for commission as Notary Public


outside the Province and cities of Cebu; that to the best of his
knowledge, no such application is pending with courts other than
this Honorable Court, and that he will not file a similar application
for notarial commission before any other Court;

7. That he has been a member of the Philippine Bar in good standing


and has not been convicted in the first instance of any crime
involving moral turpitude.

Copy of the certifications issued by the Regional Trial Court of


Lapu-Lapu City of the Clerk of Court (Criminal Division),
Integrated Bar of the Philippines Cebu Chapter, Certificate of Good
Standing issued by the Supreme Court of the Philippines - Office of
the Bar Confidant; and the National Bureau of Investigation (NBI)
Clearance are herewith attached as Annexes “E”, “F” “G”, and
“H”respectively.

8. That attached hereto as Annex “I” are three (3) specimens of his
Official Signature together with the three (3) specimens of his
Official Dry Seal.

PRAYER

WHEREFORE, it is most respectfully prayed of this Honorable


Court that Petitioner’s appointment as Notary Public for and in the City
of Lapu-Lapu be RENEWED for another 2-year period starting January
1, 2020 to December 31, 2022 pursuant to Section 13, Rule III of the
2004 Rules on Notarial Practice.
