Fatty acids:

Linoleic = omega 6 DRV 2 teaspoons of vegetable oil, 1g/day

Linolenic = omega 3 (hierarchy is linolenic, (1/30)epa, (1/16)dha) DRV 1g/day 2-3 oily fish portions/week

N.B. Examples of foods containing unsaturated fat include: soybeans, sunflower, fish, and corn oils
(polyunsaturated) and olive, peanut and canola oils (monounsaturated).

Essential fatty acids are needed for energy production, to help diffuse oxygen in the bloodstream, in the production of
haemoglobin, to lower high blood LDL cholesterol, rasie levels of healthy cholesterol (HDL cholesterol) stabilise blood
sugar levels, strengthen your immune system and reduce water retention by assisting in sodium and water removal.
Good sources of unsaturated fats include:
• Avocados (one quarter of an avocado contains 5g of unsaturated fat)
• Unsalted nuts (cashew, brazil, pecan, walnut)

• Seeds (pumpkin, sunflower, sesame).

Polyphenols, flavinoids, and catechins:
Lycopene found in tomatoes and other red / orange vegetables, as well as watermelon, becomes activated
also has an anti-inflammatory effects reducing the inflammation associated with the common cold/cvd.
Dose >2mg (unestablished).
Pycnogenal also has antioxidant and anti-inflammatory properties. Pycnogenal is found naturally in small
amounts in the skins or seeds of grapes, blueberries, cherries, and plums. Dose >25mg (unestablished).
Green tea can also have anti-inflammatory and antioxidant properties. Dose 2–3 cups/day (unestablished).
Maybe a lean tissue generating effect of green tea catechins (rat study).
Chlorophyll in plants may help the body clear toxins. 500mg spirulina/day. May also help in cancer
treatment more research needed..

Phytates and goitrogens:
Phytates in undercooked beans or pulses block absorption of iron, uncooked cereals, grains and beans.
Goitrogens block iodine (onionas, garlic, cruciferous veg.
Thiaminase in raw fish blocks thiamine.

Other supplements:
Caffeine RD <200mg/day for pregnant women. Excess = risk of miscarriage and low birth weight. Can boost
metabolism slightly but can interfere with absorption of calcium and vitamin A. 3-6g annehydrous for >2hr
exercise performance.
Acai berries are a powerful antioxidant but have no proven fat loss role.
Tumeric 3g/d or curcumin 150mg/day for health like alzheimers inflammatory disease may aid
chemotherapy. 400-600mg may aid exercise recovery (10g/d tumerc).
Alli (Orlistat) weight loss pills work by impeding the body’s absorption of fat using a statin. For every 2 lb
you work to lose through healthy eating, Alli can help you lose 1 lb more. Dose 180mg/day. Statins inhibit
an enzyme called HMG-CoA reductase, which controls cholesterol production in the liver.
Flora proactive lowers cholesterol by using wood pulp that combines with cholesterol in the bowels
preventing reabsorption across the intestinal membrane.

CoQ10 (ubiquinone) = energy + antioxidant effect. Supplements may help people with low levels of CoQ10
aged 30 years plus. Sources = meat and fish. Dose 30mg to 100mg.

Acetyl l-carnitine, abbreviated ALCAR, is derived from l-carnitine. L-carnitine is a compound which is
synthesized from two amino acids: methionine & lysine. = coenzyme in fat transport from cells and antioxidant
that can pass brain barrier. Sources = red meat. Dose usually around 2g (poorly absorbed). – little evidence for
weight loss.

5-HTP a derivative of tryptophan raises serotonin levels therefore an antidepressant, a sleep aid, and an
appetite suppressant. Dose around 100mg. Sources poultry, fish, meat, nuts.

Alpha Lipoic Acid increases energy production, can increase production of Glutathione (another
antioxidant), enhance other antioxidants, is a powerful antioxidant that can pass the blood / brain barrier,
and may help normalise blood sugar levels in diabetics. ALA can help treat Peripheral Neuropathy. Dose
100mg for cold 1800mg for cancer/cystic fibrosis, multiple sclerosis). Dietary sources not good for medical
uses.

Recognized health benefits of omega-3 prevents cardiovascular disease, promotes a healthy heart,
increases cognition including memory and concentration, helps to keep joints supple, promotes a healthy
liver, improves the condition of the skin help keeping it unblemished, helps stabilize mood, in women, krill
oil can help with the symptoms of PMS, krill oil omega-3 may help reduce cholesterol. Sources fish,

vegetable oils. Hierarchal Alpha-linolenic acid > EPA > DHA. Dose around 1g 2 -3 portions of oily fish /

week.

L-glutamine supports the immune system providing energy for lymphocytes. A further benefit of L-
glutamine is that it can aid and help maintain a healthy gut. L-glutamine can improve the barrier function of
the gut and help prevent leaky gut syndrome. L-glutamine crosses the brain-blood barrier and transports
nitrogen to the brain. Benefits include an improvement in mental function such as memory, alertness and
concentration. May help with inflammation in and training recovery. Dose 1000mg. Sources beans, eggs, fish,
wholegrains, meat, nuts.

Phosphatidylserine (a phospholipid) improves Cognition and Memory, may improve exercise performance and
recovery times and may help adhd. Dose 400mg. Sources = fish.

Saw palmetto anti-inflammatory, prostate problems. 320mg.

St John's Wort As A Sleep Aid, for pain relief, SAD, depression but can badly effect blood thinning
medications. Dose 300mg.

Taurine (made from cysteine) helps maintain muscles, helps to remove fatty deposits in the liver, reduce blood
pressure, is an antioxidant and can cross blood brain barrier, lowers cholesterol, reduces cirrhosis, acts as a cell
volumizer , mimic insulin, improved sleep and focus when exercising, and reduces muscle cramps by regulating
calcium levels which are linked to muscle cramp. Sources eggs, fish, meat, milk, seafood. Dose <5g/day.

Evening primrose oil rich in omega 3 and 6= anti-inflammatory, painkiller effect used for PMS, may help
prevent hormone imbalances, may protect against breast cancer, and may help alcoholics withdrawl
symptoms. 1000mg/day.

HMB a metabolite of the essential amino acid leucine for protein synthesis, may help muscles combat
protein breakdown, reduces LDL cholesterol, and subcutaneous fat. Dose 3g/day. Sources meat, fish.

Glucosamine 1000mg found in joint tissue aids inflammation and recovery may help in arthritis.

MSM (Methyl sulphonyl methane) 2000mg/day (cruciferous vegetables, meat/fish, legumes, eggs, onions, and
garlic) rumoured to be good for joint inflammation for arthritis in winter.

Chondroitin 1200mg/day (shell fish / cartilage) rumoured to be good for joint inflammation for arthritis in

winter.

Diet and General Disease Recommendations:

No more than 3 alcoholic drinks/week (2 in diabetes on separate days).

Gout, kidney stones excess uric acid = reduce meat, organ meat, shellfish, fish content increase: fish oil
supplement = anti-inflammatory, dark berries, have soya beans + tofu, nuts, and seeds to replace meat
protein. Tumeric increases uric acid levels too. Reduce alcohol consumption, increase exercise and most
importantly stay hydrated. Dairy consumption may reduce gout risk slightly. Egg is low in purines
associated with gout whereas sardines and anchovies, organ meats and shellfish are high in purines.
Vitamin C toxicity can cause raised uric acid levels. Check for underlying kidney disease.

Anticoagulant therapy: Alcohol in limited amounts thins the blood but can raise blood pressure and
therefore can be dangerous in people with clots in large amounts, 2-3 units a day 2-3 drinks a week is fine.
InternationReferenceNumber this is to do with how well the blood flows and needs to be consistent with
diet so the warfarin or other anticoagulant dose can be adjusted for the individual. To do this vitamin K
with meals needs to be consistent. If thin blood warfarin or other anticoagulants can be reversed by oral
administration of 0.5mg vit K (1/2 cup of cabbage or cooked spinach) takes about 4hrs. 100mcg vit K a day
is recommended in diet to keep warfarin consistent. Avoid St-Johns wart and Ginseng and Omega 3 due to
blood thinning properties. Normal warfarin dose 2-10mg. Keep vit K consistent day to day with level it was
when dose warfarin or anticoagulant calculated. Onions, garlic, artichokes and omega 3 rich fish may also
thin the blood so again needs to be consistency in the diet for an accurate warfarin dose to be calculated.
Liver is also very high in vit K.

Cold/Infections/Flu: Green, red, white tea = 8 cups/day, Garlic = 500mg/day*, Ginger 1000mg*, Echinacea =
500mg. *Anti-viral,fungal,bacterial,parasites. Vit D 30ug.

Rheumatoid arthritis: Research suggests that older women who consume more vitamin D from foods or
supplements tend to have a lower risk of developing rheumatoid arthritis 20mcg/day.

Psoriasis: vit D cream with corticosteroids not vit d deficient.

Hyper/hypo parathyroidism: Results in too little too much calcium and calcification arteries, calcium
stones/kidney stones or alternatively for hypo brittle week bones. Treat with vit D3 25ug.

Pain: Ginger 1000mg

Inflammation: ALA 500mg, Omega 3 1000mg. Joint inflammation = Turmeric 500mg (95% Curcuminoids),
Glucosamine 1000mg, Chrondroitin 12000mg, MSM 2000mg, L-Glutamine 15g/day. Arnica cream.

Cancer: Resveratrol 500mg. Eat more energy dense foods, may need to lower fibre content if stomach
upset / digestive stress, reduce saturated fat, don’t overcook meat, phytoestrogens – isoflavones e.g. soya
beans, tofu, flax etc. Supplements have poor efficacy lots of fruit and veg for vits mins = best. Omega 3 fish
oils no efficacy as a supplement all about O3:6 ratio, 1:2 certainly below 1:4 (most get 1:15). Diet should
focus on foods they like and can eat, eg may have problems swallowing. Exercise requirements same as for
healthy individuals, but just do what’s possible. Turmeric extract (95% curcuminoids) 400-600mg may
reduce cancer risk have 2 meals a week containing turmeric (may also reduce risk of alzheimers). Ca
1500mg plus Vit D3 25mcg / day reduces most cancer risk.

Blood pressure normal= 120/80-140/90 pre hyper hypertension = 140/90, low 90/60 Keep sat fat under
control (no more than 2 animal fat portions a day e.g. 1 high fat prot+1 portion butter). Sodium = 200mg in
28g of bread, ham, cheese and 400mg in 28g of luncheon sausage like salami. Flaxseed contains omega 3
and 9 and lowers blood pressure as do beets via nitrates and dairy via inhibition of angiotensin converting
enzyme (ACE) by by-products of gut bacteria digesting prebiotics.and cocoa via flavonoids and anti-
inflammatory effect. Garlic (via sulphur compounds) (500mg aged garlic extract) and Tea (Epi-3-gallo-
catechin-gallate) are also high in flavonoids. Anthocyanin rich berries may also reduce blood pressure. 25g
Soy (via isoflavones) reduce blood pressure to.

Heart Disease = Resveratrol 500mg. Vit D3 10ug may improve ejection fraction. The ideal cholesterol level
should be less than 4 mmol/l with LDL cholesterol less than 2 mmol/l. > 5 mmol/l total is high. Ideal HDL/LDL
ratio = >0.33 (> about 1/3rd). Low-density lipoproteins (LDL) vary in particle size and density, factors related
to oxidative changes and plaque formation. Larger LDL subfractions (1, 2, 3) are considered type A and are
less frequently associated with coronary artery disease. Small, dense LDL subfractions (4-7, nontype A) are
generally considered more atherogenic. There is an increased prevalence of non-type A LDL subfractions in

Type 2 diabetes and insulin resistance syndrome. Improvement from >cardio >HDL as does > mono/poly
fats and <sat+trans fat. LDL particle size can be increased by ensuring adequate vitamin D, fish oils,
avoiding excessive fructose, and refined carbs. People with familial hypercholesterolemia need to keep
dietary cholesterol below 400mg. Apolipoprotein E mutation mean LDL is very poor at carrying cholesterol
and increases risk of arterial plaques in this case sat fat reduction is very important. HMG CoA reductase is
inhibited by statins (if taken with niacin then 6% risk of muscle breakdown), also rare risk of neuropathy. Vit
D3 10mcg reduces muscle weakness from statins daily.
APOB on the LDL particle acts as a ligand for LDL receptors in various cells throughout the body (i.e. less
formally, APOB "unlocks" the doors to cells and thereby delivers cholesterol to them). Through a
mechanism that is not fully understood, high levels of APOB can lead to plaques that cause vascular
disease (atherosclerosis), leading to heart disease. There is considerable evidence that levels of APOB are a
better indicator of heart disease risk than total cholesterol or LDL. Substituting protein for carbohydrate in
the context of a healthy dietary pattern resulted in the most favourable profile of apo B lipoproteins. In
addition, compared with a typical high-saturated fat diet, healthy diets that emphasize carbohydrate,
protein, or unsaturated fat reduce plasma total and LDL apo B. Recommend mod DASH (slightly more p).
Triglycerides <1.695 normal >2.26High > 5.65 very high statins needed (conversion factor 0.011 aprox).

Type 2 Diabetes = 20ug vit D may help. Low glycemic, high soluble fiber, chromium 100mcg). Choose lower
GL starches. Also smaller meals more regularly 20-30p, 40-55c, 25-30%f satfat<10% (modDASH). GL use
nutritiondataself tracker. No meal should be high GL. 2 alcoholic drinks a week max, preferably none.
Blood sugar should be monitored and diet adapted according to individual carb tolerance. 5 portions of
fruit & veg okay with 1-2 portions nuts or seeds in diet, 7 without. More alliums, 28g dark chocolate as part
of kcal controlled diet; almonds (2oz) and peanuts(2oz)/peanut butter(3tbsp) first thing in the morning.
Chol <2.5mmol LDL. Resistance exercise important along with HIIT to lower H1ABC. Alcohol with sugar may
increase blood sugar but lots of alcohol can drastically lower it by >increasing sensitivity. High fat meals
raise oxygenated glucose due to inhibiting pyruvate dehydrogenase and preventing glucose fuelling the
Krebs cycle. Exercise 20-60mins reduced by 12.5% Hi 25%Mod 60-90mins 25% and 50% respectively. Make
sure healthy probiotic balance as lack of a good balance results in reduced short chain fatty acids like
butyrate (found in milk, cruciferous veg and wholegrains) along with acetate and propionate these are the
primary fuel for gut cells which can increase fat oxidation and reduce fat storage as well as possibly
influencing glucose metabolism. Good oral health may also help unhelpful bacteria getting into the blood
stream and travelling to the gut in the first place.
Note glucose meter 2013 standards are <5.6mmol/l within 0.83mmol/l >5.6 within 20% of true findings.
Children with Type 1 diabetes (NICE 2015) convert mg/dl & mmol/l by dividing/timesing by 18.

• on waking and before meals: 4–7mmol/l

• after meals: 5–9mmol/l.

Adults with Type 1 diabetes (NICE 2015) [non type 2 diab =<7.7mmol/l post prandial-7-7.7 may be pre diab]

• on waking: 5–7mmol/l
• before meals at other times of the day: 4–7mmol/l
• 90 minutes after meals: 5–9mmol/l.
• Driving >5mmol/l

Type 2 diabetes (Diabetes UK Council of Healthcare Professionals 2015)**

• before meals: 4–7mmol/l

• one hour post meal 10mmol/l
• two hours after meals: less than 8.5mmol/l.
• Driving >5mmol/l

Pregnant women with diabetes (NICE 2015) 5mg Folic acid (12 weeks) as high risk for birth defects

• fasting: below 5.3mmol/l also 15mins before meals

• 1 hour after meals: below 7.8mmol/l
• 2 hours after meals: below 6.4mmol/l
• Driving >5mmol/l
HbA1c – GLYCOLATED HAEMOGLOBIN (RED BLOOD CELL THAT HAS A SUGAR ATTACHED)
Normal for person without diabetes: Less than 5.7%
Official ADA recommendation for someone with diabetes: 7.0% (48mmol/l) or less

Osteoporosis = weak, brittle bones (breakdown exceeds repair) BMD<2.5 SD from mean of young healthy
adult (1000mg calcium, vitamin D 25ug, manganese 5mg, zinc 15mg, cu 2.5mg).

Osteomalacia = inadequate bone mineralisation due to poor diet (basically soft bones due to inadequate
amounts of bone minerals being lain down) (treatment as above for treatment).

Diverticular disease or IBS = diets need to be high in soluble fiber (oats, seeds, berries, nuts). FODMAPS
Kwashiorkor and Marasmus. A child with marasmus looks emaciated. Body weight may be reduced to less
than 80% of the average weight that corresponds to the child’s height. Marasmus occurrence increases
prior to age 1, whereas kwashiorkor occurrence increases after 18 months. It can be distinguished from
kwashiorkor in that kwashiorkor is protein wasting with the presence of edema.

Vitamin D deficiency = rickets, or osteoporosis risk. 6hrs sunlight = 5 times number of days (30 days of vit
D) 25% longer needed for dark skinned.
Celiac = Paleo
Vegan/Veg = Need to replace meat with meat alternative beans, pulses, nuts, tofu still need same protein
as none vegetarians.

normally becomes 2.6mch in pregnancy. Ca Ph, Mg 20% higher. Vit D may be needed to ensure no rickets.
Ginger 500mg for breast pain, isoflavones 500mg may also help (as in PMS), aspirin for BMI>35 to reduce
CVD risk duet to platlet aggregation 75mg 1/day. No shark, marlin, big game fish. Tuna trout, salmon,
sardines, mackerel, herring, 2 portions a week no more, or 4 portions (120g) of tinned tuna or salmon. No
raw egg, milk products avoid fresh restaurant mayonnaise, un-pasteurised cream cheese, camembert or
brie and all mould cheeses e.g. blue cheeses. Liver should be avoided during pregnancy and when trying to
conceive due to vit As influence on incr birth defect risk. Avoid BBQ’s and situations where food could be
undercooked. Avoid raw steak and undercooked meat. Make sure all veg is well washed in HOT water,
avoid salad bars and salad buffets. Avoid ice cream in buffets that could be defrosted and refroze. Wash
hands carefully after toilet and before meals. Paracetamol fine as normal no ibuprofen or co-
codomal/codine (codene maybe fine after first 12 weeks and before last 4). Arnica cream may help pain.
5mg folic acid if diabetes or parental birth defects as increased risk up to 12 weeks.

PMS: Ginger 500mg breast pain and cramps, isoflavones 500mg hormone related inflammation, omega 3
1500mg also inflammation. Arnica cream may help.

pre+probiotics 5000cfu/day, alpha lipoic acid 1800mg, B vitamins set to 200% RDA, 100% through diet 100%
through supplementation. Bromelain 500mg may be used to make meat easier to digest. Chlorophyll may
help take spirulina 500mg/day, iodine at 220ug as opposed to 150ug may be helpful as the thyroid is under
increased stress. +500kcal on requirements as energy absorbed les efficiency, 20% protein. Resveratrol
500mg. Eat more energy dense foods, may need to lower fibre content if stomach upset / digestive stress,
reduce saturated fat, don’t overcook meat, phytoestrogens – isoflavones e.g. soya beans, tofu, flax etc &
lignans in veg wholesgrain pulses and fruit. Diet should focus on foods they like and can eat eg may have
problems swallowing. Exercise requirements same as for healthy individuals, but just do what’s possible.
Turmeric extract (95% curcuminoids) 400-600mg may reduce skin & stomach cancer risk have 2 meals a
week containing turmeric. Ca 1500mg plus Vit D3 25mcg / day reduces most cancer risk but not breast or
colorectal. Omega 3 3000mg reduces breast skin oral risk.

Alzheimers: Omega 3 3000mg + vit D 10mcg, higher antioxidants A(1000mcg), C(100mg), E(15mg),

acid 1800mg. Vit D3 10mcg a day may help prevent. Ketogenic diet for neurodegenerative aspect of MS
where myelin sheaths degrade.

Obesity: 20-30, 30-40, 30-40% increase physical activity as to guidelines. Avoid plastinocides found in
processed packaged meats like ham, sliced turkey and in cheese wrapped in plastic if possible as these
contribute to weight gain. Good oral health, olistat as directed by Dr, green coffee extract 5000mg (50%
Chlorogenic Acid), glucommanan (500mg), garcinia cambogia 1500mg (60% HCA), CLA 3000mg. Make sure

(found in milk, cruciferous veg and wholegrains) along with acetate and propionate these are the primary
fuel for gut cells which can increase fat oxidation and reduce fat storage as well as possibly influencing
glucose metabolism. Good oral health may also help unhelpful bacteria getting into the blood stream and
travelling to the gut in the first place.
Kidney Disease: restrict protein to 8% kcal and phosphorus to <1000mg and potassium <2000mg. May need
essential amino acid supplement particularly men.

Cystic Fibrosis; more alliums, cruciferous veg, lower GL meals, reduce total sugar as there may be a link
between CF and impaired glucose tolerance (autoimmune related). Avoid refined sugars. Higher
antioxidants A(1000mcg), C(100mg), E(15mg), Se(75mcg) Zn (20mg), alpha lipoic acid 1800mg*.
(*Higher homocysteine in these conditions increases the risk of brain and CVD disease).

Migranes and Cluster Headaches; Ketogenic diet (CARBS BELOW 8%) may be beneficial.
