I OSAYINTOLA A TITUS, male, Christian, Nigerian citizen of 50 Idera Estate Oke Odo
Adigbe Abeokuta in Ogun State do declare as follows:

1. That I am the declaring herein.

2. That I am the true owner of the declared lost vehicle particulars issued by Lagos State
Government Licensing Authority.
3. That I am the true owner of vehicle Mitsubishi Jeep, model 2000 sliver in color with
registration no. LSR 643 AT, Chassis no. JA4MT31H3YP816213 and Engine no.
MWB35036672.
4. That the same documents got loss in transit as well as moving from one location to
another.
5. That the above document is required for record purposes.
6. That I swear to this affidavit to obtain replacement for the lost document.

And that I solemnly declare that, this affidavit in good faith believing same true with the
Statutory Law of Ogun State, Nigeria, 1978.

I Osanyintola T.Ayinla of Department of State Services , Oyo State Command disposed my

vehicle, Jeep Montero model 2000 with registration number LSR 643 AT, chassis number

JA4MT31H3YP816213 and engine number MWB35036672 to Alhaji Alli Murthado

Kewulabi of Surulere community Dacamca Area Odo eran Owode Oyo Township, Oyo State.

Therefore ownership and title deed of the vehicle ceased to be mine immediately and is forthwith

transferred to Alhaji Alli Murthado Kewulabi.
