SWORN AFFIDAVIT FOR LOSS OF GLO SIM CARD AND SIM PACK

I, ONI TEMITOPE EZEKIEL, Male, Christian, Nigeria Citizen of No 7, Ogunleye Street, Off Liberty
Road, Oke – Ado, Ibadan, Oyo State Nigeria, do hereby solemnly declare on Oath and State as
follows:

1. That I am the above named person whose passport photograph appears herein;

2. That I am the rightful owner of the GLO LINE with number 09059446055 that got lost
at The Polytechnic Ibadan, Unity Hostel.

3. That the said SIM CARD and SIM PACK got lost with my Phone on 26th of March, 2021.

4. That all effort made to trace it prove abortive;

5. That this affidavit is also needed for record and official purpose.

6. That I make this solemn declaration in good faith believing same to be true and correct
in accordance with the Provision of the Oaths Act, 2004.
