A RESOLUTION FOR THE CHANGE OF SIGNATORIES OF THE CPSS BANK ACCOUNT
 
Whereas, the constitution provides that the Finance committee of CPSS has the responsibility
of handling and managing the society’s funds and has the duty to collect, to safeguard, to
disburse and to make reports of the Society’s funds.
 
Whereas, the Organization sees the need to continue the operations of the CPSS Bank
Account that shall provide an effective security system of the Organization’s finances.
 
Whereas, the Organization has a new set of officers and therefore shall appoint new
signatories.
a. To sign, execute and deliver any and all applications, contracts, agreement and
instrument and forms, including, but not limited to, the merchant agreement, as well as
to submit/deliver any and all documents, which Bank of the Philippine Islands (BPI) may
require in connection with the Organization’s application for accreditation as a merchant;
b. To open an account with any branch of the Bank of the Philippine Islands which shall be
used as a settlement account, where the proceeds of any credit, debit and prepaid card
transactions, will be credited to.

RESOLVED, further, that any and all withdrawals from the organization’s account(s), including
checks, notes, drafts drawn against said bank account or accounts, as well as any other debit
instructions or transfer of funds to and from any of the organization’s account, shall bear the
signature/s of the following:
