The self assessment/review form needs to be filled in the excel sheet and forwarded to your respective Repo

Manager's / HR . No print outs of the same would be allowed.
The review forms contain confidential information and should be treated with due respect for the ind

privacy. Any violation to the same would be viewed strictly.
After the review meeting, your Reporting Manager would share his review report with you, so that you und

improvement areas needed.
The appraisal form should contain sufficient information and analysis to support the Self and Manager Rati

Incomplete forms or forms with inadequate data will be sent back for a re-review.

5 The space marked in blue is to be filled by the Reporting Manager's or Immediate superior only

6 The space marked in pink is to be filled by the Employee for self assement .

7 The last Sheet is for HR use only and not to be filled either by the individual or the manager.
