Greetings of Good Day!

I would like to inform your good office that the office of the Local Civil Registrar of
Ayase City Hall refuse to grant my request for the issuance of my husband’s
Certificate of Residency (JUMINHYO) due to the failure of my husband’s personal
appearance subjected for their strictly compliance. That only the person who
appeared personally at their respective office shall have the right and only he will
be granted to claim for his Certificate of Residency (JUMINHYO) in accordance
with the City Hall’s rule.

Thus, I fail to get my husband’s Certificate (JUMINHYO) and cannot submit any
copy as one of the requirement for my Father’s Visa Application. And one of the
reasons why I cannot ask Him to seek for his Certificate (JUMINHYO) was already
stated on my first letter which was already submitted at your respective office.

Hope that this letter be guided respectfully for your kind consideration.
