Dear Sir Nuñez,

Christian greetings! My utmost respect to you Sir.

Let me introduce myself first. I am Keith Wong, previously employed as one of


Emergency medical provider/Ambulance Driver. I encountered a vehicular mishap
sometime in May 22, 2020 during and in my performance of duty at Dalipuga, Iligan City.
With this, my Driver’s License was confiscated by the Traffic Division of Iligan City.
Accordingly, the aforesaid division will only release my license upon the request of my
immediate head, as in this case Sir, you as the head of the ICDRRMO.

With this, I am respectfully and humbly writing this follow up letter regarding my
Driver’s License which was confiscated by Iligan City Traffic Division in May 22, 2020. I
badly need my license Sir so that I can search for a job amidst this pandemic wherein I
need to help provide for my family.

Sir, thank you for your kind consideration regarding this matter. I am hoping for a
positive response from your good office.
