Note for Admitting Staff


1. Refrain from requiring Affidavit of Discrepancy to establish the identity of the person,
the same can be satisfied by providing the required document.
2. Baptismal Certificate cannot be substituted as a valid ID for purposes of satisfying the
requirements, it is a mere proof that the event took place.
3. More lenient for correction that can be supported by PBEF, this will entitle the patient
for discount in his/her bill.
4. If the photocopy of the document such as Birth Certificate, Marriage Contract or IDs
seems to be tampered, the admitting staff may require the presentation of the original
document for verification. If the document is tampered without the authorize signature
refuse to accept the document as proof of identity.
Valid IDs for purposes of correcting entries includes but not limited to:
1. Passport
2. Senior Citizen
3. ITR (BIR)
4. Voter’s ID/ Voter’s Certification
5. SSS/ GSIS UMID ID
6. Postal ID
7. Driver’s License
