PARENT OR GUARDIAN’S DECLARATION

I hereby declare that the information and the documents provided in this application by my
son/daughter/ward is true and correct. I understand that if any information is false or the
supporting documents provided by me or my ward is found to be false/forged, then we are
liable to return the entire scholarship amount received by my ward with interest at 18 %. I
understand that in addition we are also liable for appropriate legal action both civil as well as
criminal for such an act. My ward will also be removed from FFE’s scholarship program.
For the support that my son/daughter/ward is receiving from FFE, I undertake and pledge
that my son/daughter/ward will support the mission of FFE by contributing in one or many
ways identified in the Pledge signed by my son/daughter/ward.

I also undertake to keep FFE informed about the address and employment status of my
son/daughter/ward at all times.
