SUBJECT : Report on the Status of PSMS Decelyn S Daguro, Field

Training Officer at Koronadal CPS

2. This pertains to the status of PSMS Decelyn S Daguro, Field Training Officer
deployed at Koronadal City Police Station and presently confined at Allah Valley
Medical Specialist, Inc., Koronadal City.

3. Please be informed that at about 8:00 o’clock in the morning of May 27, 2020,
PSMS Decelyn S Daguro rendered her duties as Field Training Officer (FTO) to the
PSFTP CL 2020-01 police students presently deployed at Koronadal City Police Station.

4. At about 11:30’clock in the morning of said date, PSMS Daguro, together with
her fellow duty FTO, PSMS Ma. Carolina S Porras, went to their billeting quarters
purposely to have their lunch thereat. While they were resting at their room, PSMS
Daguro felt severe stomach pain. Since the pain was too unbearable, PSMS Daguro
requested PSMS Porras to inform her husband, PSMS George Ian G Daguro, about her
physical condition.

5. At about 1:00 o’clock in the afternoon of same date, PSMS Daguro was
confined at Allah Valley Medical Specialist, Inc. Koronadal City with initial diagnosis of
Urinary Tract Infection. She was further diagnosed to have fatty liver and cysts issues.
Accordingly, she is tentatively scheduled for operation on June 1, 2020. Progress report
will follow.
