3. Wait to speak until given permission to do so
4. Always seat yourself at a table within 5
minutes before start bell rings, and remain
seated until after the end bell rings.
5. Accept the consequences of breaking the
following rules or otherwise disrupting the
classroom.

For any notable instance of excellent behavior, two scoreboards in the teacher’s records will be filled: one
will tally points redeemable toward individual student rewards based on that student’s excellent
behavior, and the other will tally points redeemable toward rewards for the entire class as a group for
overall excellent behavior. For each of these scoreboards, we will call the points “WitCoins.”
Fifth Infraction: Meet again with parents and/or principal, loss of two
Group Infraction: 3-strike warning system, loss of one on third strike, lose
classroom choice of one group reward type (Group reward 1, 2, or 3) on each
further strike.

The student for whom this form was issued is to come to the teacher’s classroom during his or her 15
minute break and fill out the following:
1. What did I do wrong to earn this form and other delineated consequences?
2. What should or could I have done differently that would not be in violation of the
rules?
3. What will I do better next time?
I believe in each student and their ability to do and be good. Whatever circumstances that brought you to
conduct the above inappropriate behavior, I will give you an opportunity to make it better (i.e., apologize
for an insult, clean up a mess/marking you purposefully made). Please write what you plan to do below
and if I believe it will make the situation/environment better than before the bad behavior, I will allow
you to do it and reward you one .
