was absent in the office from 27 Sep –29 Sep to 04 Oct –06 Oct due to
serious back pain & fever. Doctor has advised me to take rest for certain
period. Prescription of doctor is attached for your kind ready reference.

Under the above circumstances I request you to kindly allow me 06 days
