A. Original or Authenticated Latest Semestral Report Card (Certification of Grades)

with corresponding units indicated with school seal (2 nd Semester SY 2018-2019)
not less than 18 units. School must indicate the exact equivalent grade of
numerical rating issued by the registrar (ie.1.5-90.00%).

B. Original or Authenticated Certification of Academic Load Enrolled issued by the

school registrar (1st Semester SY 2019-2020) not less than 18 units.

C. Latest Military Science grade duly signed by the Chief Clerk and Commandant or
grades from the school registrar (2nd Semester2018-2019).

D. Equivalent Points for the Aptitude, Bearing and Service Requirement.

E. ROTC Cash Incentive Performance Data Sheet.

F. Computation of the Grade Point Average.

G. Bio-data with two (2) copies 2x2 ID pictures.

H. Duly Accomplished Engagement to Serve.

I. Security Declaration.

J. ROTC Cadet Cash Incentive Contract/Undertaking duly signed by the applicant

and notarized in a Notary Public.

K. NSO Birth Certificate.

L. Physical Fitness Test Result.
