UTILIZATION OF UPDATED HRD ISO FORMS FOR LEARNING AND

DEVELOPMENT (L&D) ACTIVITIES AS PER GUIDELINES

Public Schools District Supervisors/District In-Charge
Public Elementary and Secondary School Heads
All others concerned

1. This Office, through the SGOD-HRD Section hereby informs every one of
the adoption of and compliance to the updated HRD ISO Forms for all L&D
activities initiated and conducted by SDO-based employees and the field.

2. It is expected that all L&D proposals and terminal reports starting

Monday, January 20, 2020 shall utilize the said forms. (See attached)

3. In reiteration, ALL L&Ds shall utilize the forms including meetings and
among others, ranging from 1-day duration onwards. The registration
form shall be utilized prior to the conduct of the L&D should the
proponent/ s need to account the prospect total number of attendees, while
the attendance form shall be accomplished during the implementation of
the approved L&D.

4. For Division level L&Ds where the SDS is the signatory, the standard
certificate template shall be used (Refer to DM 3, 2020). Proponents are
advised to secure from the Division ICT Unit the certificate registry
number prior to printing.

5. Non-compliance to the herein provisions shall mean disapproval of the

activities, hence non-inclusion to the L&D Calendar/Master list of the
Office, which serves as the testifying document that the L&D is du1y
approved and official.

6. The Office's L&D guidelines is reflected in DM 142, 2019, where minimal
revisions in the content are already captured in the herein forms.
