    Elementary & Secondary School Heads
    Teaching & Non-Teaching Personnel
    All Others Concerned

    1. The Office hereby reminds all employees the required documents to be


    submitted to the Bureau of Internal Revenue - Revenue District Office No. 2, as
    follows:

    a. Scanned copies of BIR Form No. 2316 {in PDF) saved in a CD, using
    the filename: Taxpayer's Surname_TIN_12312021 (ex: DELA
    CRUZ_l23456789_12312021) and CD label: Name of School; and

    b. Duly notarized BIR Transmittal (by school) with documentary stamp


    (see attached format).

    2. The deadline of submission at the Bureau of Internal Revenue - Revenue


    District Office No. 2, Bantay, Ilocos Sur is February 28, 2022. Hence, all employees
    shall take note of the deadline to avoid penalty for the late submission.

    3. For SHS and ALS mobile personnel, they shall be included in their respective
    schools. For SDO Personnel, please submit your duly accomplished BIR Form No.
    2316 at the Records Section.

    4. For monitoring purposes, school heads are requested to submit one (1)
    photocopy of the BIR transmittal, duly stamped-received by the BIR and one (1)
    hard copy of the BIR Form No. 2316 at the Records Section.
