Dear CEO Name

With reference to the letters dated [date you sent letter 1] and [date you sent letter 2] delivered to
your by Royal Mail (Recorded post or proof of posting obtained). In those letters I asked you to provide
the following reasonably requested specific items:
1. A copy of the relevant and lawful Terms and Conditions;
2. A true and certified copy (Not photocopy) of the Deed of Assignment (NOT Notice of
Assignment);
3. A true and certified copy (NOT photocopy) of the Original Note (Credit Agreement);

I hereby serve Notice that failure to provide Proof of Claim by [Company Name], has created a
permanent and irrevocable estoppel by acquiescence, forevermore barring [Company Name], and it's
agents from bringing any claims against [Your Name]. I hereby declare that the rule of agent and
principal shall apply and that service upon one is service upon another.

Furthermore, YOUR failure to provide Proof of Claim in the form of the above reasonably requested
specific items comprises [Company Name Here]’s, agreement to the following terms and conditions -
that the debt did not exist in the first instance and/or it has already been paid in full; and that you will
be held liable for any and all damages to me incurred by your actions. Further that any negative
remarks made to a credit reference agency be removed forthwith and you will no longer pursue this
matter.

Please also note that any further invalid claims by [Company Name Here] or any of its affiliated agents,
parent companies, proxies, etc against [Your Name], and/or attempting to contact [Your Name] will
also constitute Harassment under Section 1 of the Protection from Harassment Act 1997 and the
Administration of Justice Act 1970 S.40 and against FCA guidelines which may result in legal action for
compensation for the anxiety and any financial loss caused.
