AFFIDAVIT OF CONSENT

I, MARICEL VENTIC LINOGON, Filipino, of legal age, and a

accordance with the law hereby depose and say:

1. That I am one of the children Spouses Carlito Dormindo Linogon and

Alfreda Ventic Linogon;

2. That I am aware that my parents are intending to adopt my niece, MA.

JHUDIEL CORAZON LINOGON, who is a daughter of my sister Mary Ann
Linogon;

3. That I am giving my full consent to the said adoption of MA.

4. That I am executing this affidavit to attest to the truth of the foregoing
