SPECIAL POWER OF ATTORNEY

KNOW ALL MEN BY THESE PRESENTS:

I, LOURDES ABARQUEZ JUMANGIT, of legal age, married, with residence and postal
address at Abucayan Norte, Calape, Bohol do hereby APPOINT HEIDELIZA YTIS BATULA,
likewise of legal age and married, with postal address at Mandaug, Calape, Bohol as my Attorney-
In- Fact and my true and legal representative to act for and in my name and stead and to perform the
following acts:

1. To REPRESENT on my behalf as the nearest kin of the decedent in claiming or

requesting for the release of the Death Certificate in the Local Civil Registry of
Municipality of Calape, Bohol, of my sister MARIA NIEVES ABARQUEZ YTIS,
who died on August 26, 2020 at her house located at Mandaug, Calape, Bohol;

2. To PROCESS and REPRESENT on my behalf in transacting with the SOCIAL

SECURITY SYSTEM (SSS), with regard to the BURIAL and other benefits that my
sister is entitled in the said agency;

3. To PROCESS and REPRESENT on my behalf on all transactions relating to the death

of my sister in any agency, private or public;

4. In the event that the said entitled benefits shall be ready for claiming or release, the
herein attorney-in-fact shall be my representative; and

5. To DO such any other act or thing that may be required or incidental to carry out
effectively the provisions and the purposes for which this authority is hereby given;

HEREBY GRANTING unto my representative full power and authority to execute and
perform every act necessary to render effective the power in the claiming and or requesting of
the aforementioned document with the concern agencies or offices of the government or in
any private offices, as though I myself, have so performed it, and HEREBY APPROVING
ALL that she may do by virtue hereof with full right of substitution of her person and
revocation of this instrument.
