We, FLORENCIO PAMEN BOLANIO, ARIAN DURAN SAYNO, RANDY RUSTRO,

MONABEE ROSAL, ANECITA CARACAL DACUP, SHERRY GIL MADROFIO BALDIVINO
and CHERYL LOVE TOLIBAS DAGOC, all of legal age, with office address at #70 San Rafael
St., Kapitolyo, Pasig City, after being sworn to in accordance with law, do hereby depose and
state that:

1. We are the Board of Directors of ASIAPRO TRANSPORT SERVICES a cooperative

duly organized and existing under virtue of laws of the Philippines, with principal
office address as stated above;

2. We have personal knowledge that there is no Accreditation Fee being charged or

collected by OTC from the acceptance and processing of documents for the
accreditation of our transport cooperative;

3. We have personal knowledge that OTC does not charge fees for its services to the
transport cooperatives such as renewal of Certificate of Good Standing (CGS),
endorsement of transactions with Land Transportation Office (LTO) or Land
Transportation Franchising and Regulatory Board (LTFRB) and other similar
transactions with other government agencies and private institutions;

4. We are aware that approval of our accreditation is not a guarantee or assurance that
the cooperative’s application for the issuance of Route Measured Capacity (RMC)
and/or Certification of Public Convenience (CPC) to operate our unit/s will be granted
as we likewise attest our personal knowledge of the fact that the issuance of such
accreditation is never equivalent nor an assurance to the grant of a franchise.

5. We are executing this Affidavit in order to attest the truthfulness and veracity of all
the foregoing statements.
