2 Year Well Visit

We are sending this e-message prior to your well visit to alert you of things to consider prior
to your visit. We want to customize this appointment for your concerns and questions.

To make the most of your visit, please complete any surveys in the portal prior to
your appointment.

"Homework" before your visit:

 Remember to bring your child's vaccine record so we can update it for your records.
 Please bring your child’s insurance card to all visits.
 Check insurance coverage if you have additional concerns that are not well child
topics.
 Create a list of questions to discuss at your visit.
 If you have a new phone number or address, please complete our Patient
Information Sheet, found on our Forms Page.
 Please bring any medications and vitamins you routinely give to your child to your
appointment.
 Remember to bring your payment card (debit, credit, or health savings card) to your
appointment. See our Financial Policy for more information.

Any Questions?

 Be sure to read the 2 year section on the Newborn and Well Child Care page of our
website to review topics to be covered at your visit (safety, development, diet, and
vaccines) to stimulate questions you might have and anticipate questions we might
ask.
 If your child has not had the recommended anemia and lead screening, please do so
before your visit if possible. Call our office if you need another order faxed to your
lab.
 Vision screening is an important part of your child’s well visit. This will be done at
this visit and sent to your insurance company with the CPT code 99177.
 Our office offers fluoride varnish to all children who do not yet visit a dentist but
have at least 4 teeth. This will be sent to your insurance company with the CPT code
99188.
 An autism screening is recommended at 18 and 24 months. We will do the MCHAT
at this visit. This will be sent to your insurance company with the CPT code 96110.
If you have developmental concerns, please write them down before your visit.

Have you moved? New phone number?

 Each year we must update your Patient Information Sheet to be in compliance with
insurance regulations. If you would like to complete this prior to your visit, print it
off from this page and bring the completed copy to save time in the office!

General Appointment Information:

 Thank you for your trust in our office with your child's healthcare needs. If you will
need to reschedule any appointment please call the office 48 hours before your
appointment, at 913-888-4567. Missed appointments and appointments cancelled
with less than 24 hours notice may be subject to a charge.
