AFFIDAVIT

That I, Ma. Elizabeth T. Sedillo, 35 years old, Filipino, married, and a resident of
Poblacion 2, Tanjay City, Negros Oriental, after having been sworn to in accordance with
law, depose and state that:

1. On or about 7:00 o’clock in the evening of April 17, 2014, Roselyn Balbuena
accompanied by her mother, Ma. Lorebel, and DSWD Representative, Christy
M. Mendez, went to my residence

2. I was ask to conduct a medical examination on Roselyn Balbuena in relation to her

complaint of rape

3. I proceeded to examine Roselyn Balbuena;

4. I prepared the medico-legal certificate herein attached as Annex “A”;

5. I executed this sworn affidavit to attest to the veracity of the findings as stated on
the medico-legal certificate.

IN WITNESS WHEREOF, I have hereunto set my hand this April 25, 2014, in the
City of Tanjay, Philippines.
