1. To transact with the DSWD and other government agencies

regarding the grant of Unconditional Cash Transfer (UCT) due my
deceased father _______________________ who died on August 11, 2020;

2. To sign papers, acknowledgment receipts and other

documents needed for the release of the abovementioned UCT;

3. To receive the funds and perform any other act consistent

with this power of attorney.
