Introduction including overview of area inspected and activities taking place

For practical assessment of a workplace, an HSE inspection visit to OGDCL Noshpa Well Head was
made. The purpose of visit was to identify hazards and check the compliance of OGDCL HSE
management system and identify any areas of improvement in it.
OGDCL Pakistan has a very successful E&P track record of operations in Pakistan and mission to have
LTI free environment. This report follows an inspection of the Construction site of Noshpa Well Head
Facility and Fabrication yard located 27km from Central Processing Facility (CPF), in District Karak,
KPK to meet the requirements of the NEBOSH IGC 3 practical application.

The premises are occupied by up to 135 to 145 workers including HSE officer, medic, and in charges,
foremen, welders, riggers, fitters, masons, general labours, Excavators, dumpers and Forklift Truck
during day shift only. The size of fabrication yard is about 50X30m and used for welding, cutting,
loading and unloading activities.

The Well Head construction comprises of three main discipline i.e. Civil, Mechanical and E&I. The
construction works were carried out only in day light with some extended hours for special jobs only.
The total area of Well Head was 22500 Sq.m and four major areas were inspected and relevant
hazards and good practices were noted and identified in this report.

Executive Summary OGDCL Pakistan has a very successful E&P track record of operations in
Pakistan and mission to have LTI free environment. This report follows an inspection of the
Construction site of NOSHPA Well Head Facility and Fabrication yard located 27km from Central
Processing Facility (CPF), in District Karak, KPK to meet the requirements of the NEBOSH IGC 3
practical application.
The premises are occupied by up to 135-145 workers including HSE officer, medic, and in charges,
foremen, welders, riggers, fitters, masons, general labours, Excavators, dumpers and Forklift Truck
during day shift only. The size of fabrication yard is about 50X30m and used for welding, cutting,
loading and unloading activities.
This report focuses on main four areas of concerns where improvements and implementations are
required which are summarized below.

Civil Works Hazards


Manzala-11 Well Head is already commissioned temporarily and Fire risks arise from the gas leakage,
striking with buried gas pipe lines and power cables. Deep excavation of Knock out Drum (KOD)
(below 4 feet is considered as confined space) is also hazard of equipments/machineries or workers
falling. Dry and wet cement has severe consequences to workers health and environment. All these
can be managed safely by proactive action of assessment and use of proper PPEs.
Storage and Handling of Materials
Due to limited size and haphazard lying of materials and cables, store has severe threat to tripping
and material handling. Frequent manual handling of load was observed which may lead to back pain
or other injuries. Proper machines handling techniques can solve the problems.
Welding and Fabrication Yard
Haphazard lying of tools and equipments and workers ability, entry and emergency exit were
blocked by materials. Segregation of gas cylinders, hazardous and non hazardous materials can
relieve the hazards. Proper inventory and MSDS of materials solve the hazards associated with
materials.
Mechanical and Electrical Work
Poor training and competency of workers working in Live Christmas tree, gas pipes, confined space
and working at height are severe threats to personal injuries, permanent disability or death.
Provision of special PPEs, training and supervision can solve the issues. Well defined emergency
evacuation plan and fire fighting equipments should be available and implemented.
This basic aim of this report is to highlight some specific observations that need to be addressed
immediately. Furthermore, these serious concerns leads to the breaching of Health and Safety at
Work Act (HASWA), the Management of Health and Safety at Work Regulations (MHSWR), the
Manual Handling Operations Regulations 1992, the Regulatory Reform (Fire Safety) Order (Fire
Regs), the Provision and Use of Work Equipment Regulations (PUWER), the Personal Protective
Equipment at Work Regulations (PPE Regs), the Electricity at Work Regulations and the Workplace
Regulations. Any of these breaches may lead to possible enforcement action from the HSE.

Main findings of the inspection


1- Striking with buried Services lines and Power cables
As the Well Head is already commissioned by OGDCL trough rental facility and there were numerous
fire hazards arising from poor workmanship, leakage from existent Christmas tree, striking with
buried gas lines and power cables which may lead to property damage, disability and even life loss .
No proper PPEs were used for dealing with underground activities which is itself great hazards.
The above unsafe findings represent a possible breach of the Occupational Safety Convention (C155)
and Occupational Safety Recommendation (R164) of International Labour Organization (1981).
In the short term, Management should provide underground pipe and cable detector machine and
proper PPEs to all worker involve in excavation.
In the medium term, the site management should improve control and supervisions of working staff.
Arrangement of trainings and incentives schemes should be launched for good practices.
The estimated immediate cost would be US$3400 for special PPEs and pipe/cable detectors etc.

2- Pedestal and Vehicle Operations


Pedestal and vehicles routs were not identified and segregated at main gate which may lead to
accident. Chain Excavator and tractor were not certified and operators have no required licence.
Safety reverse horns, flash beacons and banks man were also not present.
The above violation of HSE represents a possible breach of the Management of Health and Safety at
Work Regulations 1999 (MHSWR) and PUWER issues.
These issues can be solved immediately by designing proper routs for pedestal and vehicle
movements and arranging reverse horns, flash beacons and banks man for each vehicle.
In the medium term, the site management should ensure the certification of the vehicles and
arrangement of suitable drivers/operator.
The estimated immediate cost would be no more than US$1200 for purchasing of flash
beacons/reverse horns and vehicle certification.

3- Inappropriate Storage Design and Handling of Materials


The space for storing and handling construction materials was very limited. Racking was not done
properly and may cause severe injuries if collapse. Materials were placed haphazardly which have
severe consequences of tripping hazards and obstruction to emergency exit rout. Repetitive jobs of
cement bags were carried out without follow the safe procedures of safe manual handling.
The above unsafe findings represent a possible breach of the Manual Handling Operations Regulations
1992, the Employer’s obligation Article 10 (a), R 164 of ILO and employee’s obligation, article 19, C
155.
In short term, the site management should ensure the safe design of stores which is capable to
accommodate the current materials. Meanwhile the store is cleared and arrangement of extra
racking for storing of haphazardly materials.
The estimated cost for additional racking and training should be within US$ 4200.
4- Hot Jobs in Fabrication Yard
Majority hot jobs were carried out in fabrication yard. MSDS of hazardous substances were not
present and hazardous and non-hazardous materials were not stacked separately. Gas cylinder was
not segregated and protected from other materials which have potential hazards of fire. Few
welders and fitters were not competent and using wrong tools. One welders and two fitter were
carrying activities of welding, grinding and cutting without using fire resistant blanket/canopy as
these are potential sources of fire risk. Few extinguishers were present but they were not
maintained properly. Guard of one grinding machine was not present which may leads to severe
face/hand injuries.
This may also lead to the breach of the Management of Health and Safety at Work Regulations 1999
(MHSWR) and PUWER issues.
In short term, the management should remove extra gas cylinder which are not in use from yard and
store separately with proper hard barricading. The guillotine is removed from service and not used
till guard is replaced.
For medium term, behavioural training of workers is required. Proper risk assessment and job safety
analysis would also be required.
The estimated cost for gas cylinder separation/racking and barricading may be within US$ 3500.
5- Deep Excavation
Workers were pouring concrete in deep excavation of Knock Out Drum. The atmosphere was not
checked and embankments were not strengthened. They were susceptible for falling objects from
workers working on the top of KOD or workers may fall in to the deep excavation which may cause serious
injuries of legs/arms or head etc.
This may also lead to the breach of the Management of Health and Safety at Work Regulations 1999
(MHSWR), Occupational Safety Convention (C155) and Occupational Safety Recommendation (R164)
of International Labour Organization (1981).
In short term, the management should ensure use of collapsing preventing techniques i.e. cement lining,
battering, shoring and trench boxes etc and area should be enclosed with proper hard barricading, warning
tape and safety signs.
For medium term, behavioural training of workers is required. Proper risk assessment and job safety
analysis would also be required.
The estimated cost for Cement lining, shoring and barricading may be within US$ 2500.

6- Emergency Rescue Procedure and Arrangements


Permanent Well Head construction in live Christmas tree is very critical task and needs extra care
and efforts but here I observed that no attention was given to emergency rescue arrangements, fire
and gas detector instruments. Hot jobs were carried out without monitoring of atmospheric gases
through proper certified monitoring devices. PTW was present but procedures of SSW were not
followed. There was no communication b/w HSE and execution team. Fire marshal was also not
present. Electrical cables and flexes were creating tripping and hindrance to general workers. The
specify PPEs for confined space working and working at height were also not available. No
emergency evacuation plan was introduced and workers were unfamiliar about the emergency
situation. Poor Fire fighting team was available but having lack of resources.
These represent possible breaches of Article 10 (a), R 164 of ILO and ILO‘s Occupational Safety and
Health Management System (ILO-OSH 2001) as well as Pakistan Factory’s Act 1934.
In short term, the management should arrange calibrated LEL meter for monitoring the atmospheric
gases and ensure Emergency Response Team (ERT), start emergency drill and practice of evacuation
in case of emergency.
For medium term, special training about “Rescue and Evacuation” should be arranged for all
workers. Proper risk assessment, emergency drill, SSW, special PPEs and job safety analysis would
also be required.
The estimated cost would be within US$ 7900 for training and special PPEs.

Conclusions
The most significant hazards identified in this inspection relate to Fire, pedestal/vehicle crossing, fall
from height, ergonomic, accident, personal injuries and property loss. Among all hazards, emergency
response team is highly critical and need urgent attention. Many of the hazards found were as a
result of a lapse in general Health and Safety management and some of them due to personnel
behaviours and implementation of SSW. Majority of the observations could be rectified by simple
training like “tool box talk”, use of calibrated tools, maintenance and vigilant supervisions. This is a
very low cost practice which in turn could improve the Health and Safety of the workforce. However,
few issues needed immediate management commitment by providing training and safe/suitable
equipments to his workers.
Although there were good practices i.e. Dedicated First Aid Team, Good Housekeeping and incentive
schemes etc however there were various breaches of health and safety law which if could not
attend, would lead to enforcing action by enforcing agency. The remedial cost may be few thousand
dollars which is less than the total cost of loss of production, personnel disability and injuries etc
caused by an accident.
The recommendations made in this report will significantly reduce the potentials of
incidents/accidents and decrease the cost associated with incident/accident and ill health. This will
indeed improve the morale of workforce and in the long run save money from unnecessary claims,
losses of production and also improve the corporate image of the organization.
