Introduction including overview of area inspected and activities taking place

The report follows a health and safety inspection carried out by me at STAR
Constructions. The company has taken an additional contract of constructing three multi
storied buildings.

I visited the construction site at Kakkanad, Ernakulum district on 31 March 2016 at 11 am.
The construction site is for flat of 22 double bed rooms, flat of 23 bed room and 32 bed rooms.
It is a 9 store building construction with 2 lifts 2 emergency exit and two gates at front and
back side of each flat. There is one swimming pool and one gymnasium on each flat to
construct at south eastern side of the flats.

There are around 150 people working. The activities going on are
excavation, piling, scaffolding work, steel work, painting, concreting, piping, and carpentry
work etc. There are three mobile cranes and two tower cranes, three excavator, four dumping
truck and two forklifts are in operation. Also four cement mixing machine, two welding
machine and other portable equipments are using.

Executive Summary

A thorough inspection was conducted all over the site and accommodations area. Major
hazards have been identified during the site inspection. It is clear that management take
necessary consideration for the areas like excavation, crane operation, electricity, Gas
cylinders and during my inspection I did not find any work related accident or serious injuries.

A safety survey, safety tour, safety audits and safety inspections will reveals the lack of
attention by the management in the area.

The company take immediate actions to consider the excavation safety, electrical safety,
crane operation safety, fire and explosion hazards and working at height safety.

Recommendations for improvement in necessary consideration area were less expensive

(around Rs 6,25,000) and it will reduce the rate of accidents. If the company take these less
expense as a consideration then they can avoid the loss high cost by major accidents occur.
Another important factor is that the company consider these areas a requirement and accept
the guidelines of ILO – OSHA and also accept the recommendations then the rate of accident
can reduce into zero. Then the good reputation and image of the company will go high and the
company will get more business.

Main findings of the inspection

1. Electrical safety (Observation no:8, 12, 17 )

In my inspection observed welder using defective welding holder and damaged

welding cables. Also find hazardous arrangements in site office, one exposed electrical
cable which was used for photocopier laying in walkway. Electricity itself it is a hazard
which can cause electrocution, serious tissue burns, heart fibrillation, death. A small
electrical spark turns into major fire.

I recommended that defective welding set (power tool) must be replaced and
follow work permit system. All electrical equipment’s should be tested and certified
prior to use. All electrical cables are routed by ELCB. Periodical inspection should be
carried out for fixed and portable electrical equipment’s

Violation and breach of the Kerala electricity act 1904 and BOCW act 1996

The cost is around Rs 55000

2. Crane operations safety (Observation No: 3, 11, 16)

In my site inspection identified that one mobile crane outriggers placed in an un even
surface during the crane operations, also noticed few workers were walking under the
suspend scaffolding materials whilst lifting with tower crane. This shows the
management commitment towards the crane operations.

I recommended company should provide a competent crane operator, also

qualified rigger controller with proper communication system like radio, wireless
phone to be provided. Proper barricade, signage and warning alarm provided for lifting
materials. These incident can cause serious accident, company should follow safe
operating system and permit to work system

3. Handling and storage of flammable gas cylinders (Observation No:7, 10, 20)

In my inspection observed acetylene gas was leaking from the main connections, also find
one worker was rolling the acetylene cylinder. Few workers were smoking closely to the
acetylene cylinder storage area. These points are high risk it may leads to serious injury and
damage to the property.

Find exact locations of leak by using soap solution test and change the valve and coupling if
possible or tighten. Ensure that workers are trained for handling and usage of gas cylinders
and ensure that gas cylinders storing in a safe location with proper signage and fencing.

4. Excavation safety(Observation No:9, 18)

There are two excavation (20 X 10 meters) carried out near the pedestrian and vehicles moving
area. The chance of fall of personnel as well as vehicle is very high. If they follow the same
condition, there could be a major accident and it can cause serious injury and damage to
vehicles. Lack of trench protection increase the risk of cave-in which leads to trapping and
crushing of persons.

I recommended that immediate hard fencing and signage should be provided and side wall
protection like sloping, benching, shielding to be provided, Also company should follow
permit to work system. Excavated soil should be 1 meters away from the edge of the
excavation.
It is the breach of breach of BOCW act section 20.81 and 20.85.
The cost will be around Rs 2, 50,000

5. Working at height safety (Observation No: 1, 6, 15)

In my inspections observed that in different locations employees working at height

without any safety measures. People working on unsafe, unsecured platform without wearing
safety harness at the outside of 6 th floor and unsafe use of portable ladder and also unsafe
scaffolding erection.It may leads to serious accidents like people falling from height or
materials falling from height. It can cause serious injuries and fatalities.

I recommended that to conduct adequate training programs to all employees

regarding working at height safety. Provide proper PPE like safety harness to all employees
who ever Woking at height and provide safety catch nets underneath of such platforms.
Appoint a competent person for proper inspections.
It is the breach of BOCW ACT 1996 and work at height regulation 2005 regulation 5 and 6(5)(b)

The cost is around Rs 1,00,000

Conclusions

Over all the workplace was clean and well maintained in good working
condition. The main findings of the workplace were regarding unsafe excavation practice,
Electrical safety, unsafe crane operations, handling and storage of flammable gas cylinders,
and finally working at height. Inspections, commitment of the management, Instruction,
Information and training which plays an important role for correcting all the findings. So it is
necessary to focus on certain parts like,

 Providing proper inspection and supervision of the excavation areas.

 Giving adequate training to the crane operators, banksmans, those who are handling
or working with flammable gas cylinders and working with electricity.
 Appointment of competent person for performing the each and every task.
 Conducting more awareness programmes on health and safety culture.

The recommended actions require some few lakhs of rupees which is hardly a minute cost
compared to cost lost of a major accident. Implementation of the recommendations improves
the workers moral which will result in a better production with healthy environment. Also it will
helps to avoid the leagel enforcement actions against the company.
