Background:

On 10.11.2015 at 1130 AM, LPO was performed in the lab, for observing the handling of Lab instruments
& glassware. The main purpose of this LPO was to assess the risk of hand safety in lab activities, one of
the lab employees (FTC) was working with an instrument (Viscosity Bath) and had to transfer & wash
few glassware for this activity, thus he was subjected to handle instrument's temperature, glassware
and cleaning solvent. A thorough risk assessment was therefore done in order to address the criticality
of this activity pertaining to hand safety.

Positive comments:

*Observee wore all required PPEs i.e. Lab coat safety goggles, safety shoes & nitrile gloves.

*Observee briefed well about the risks associated with this critical activity and he checked that all
nearby safety devices are present at their designated place and in proper condition. i.e. Spill Kit & Fire
Extinguisher.

*JLA – Handling and Storage of Lab Chemicals (WLBP/JLA/002), JLA - Handling of Glassware (1005896) &
JLA- Viscometer Calibration (1005942) were available in the lab, being reviewed by the Observee prior to
the start activity.

*Washing sink was tidy & clear of any glassware inside it.

*Checked glassware for slippery & cracked surface.

*The work area was tidy and clear from slip/trip hazard.

*Sit/Stand stool was kept away from the walking path.

* LPSAs performed & explained by Observee,

Assess: What could go wrong if correct PPE for hands are not utilized in lab activities?

Analyze: Can cause injuries & burns to the hands if exposed to these hazards, how to eliminate/reduce
the risk?

Act: Following glove compatibility matrix & with and use appropriate PPEs can reduce hands safety risks.

Assess: What could go wrong if correct PPE for hands are not utilized in glassware/hot instrument's
handling?
Analyze: Can cause cut/injuries & burns to the hands if exposed to these hazards, how to
eliminate/reduce the risk?

Act: Use of appropriate PPEs can reduce hands safety risks.

Observee reviewed following JLAs, Handling and Storage of Lab Chemicals (WLBP/JLA/002), Handling of
Glassware (1005896) & Viscometer Calibration (1005942) however "Gloves Compatibility Matrix" is not
incorporated in JLAs, which is an standard document in selection of appropriate gloves relevant to the
activity.

He reviewed the Hand safety checklist, stated the potential hazards in this activity & selection of
appropriate gloves.

As this activity involves use of solvents, handling of hot glassware & sometimes may have exposure to
cracked or broken glassware so the specific and relevant gloves are therefore necessary to adopt.

Assess: What could go wrong if correct PPEs are not selected while washing glassware? Analyze: May
expose hands to chemical reaction, Act: Use of appropriate PPEs can reduce hands safety risks.

Assess: What could go wrong if correct PPE for hands are not utilized in glassware/hot instrument's
handling? Analyze: Can cause cut/injuries & burns to the hands if exposed to these hazards, how to
eliminate/reduce the risk? Act: Use of appropriate PPEs can reduce hands safety risks.

He demonstrated good awareness of the hidden risks if he removes his gloves before the completion of
task.

He inspected the integrity of glassware prior to start activity, was well aware of the risk if any broken or
cracked glassware is handled without wearing gloves, it may have severe cuts on hands.

He briefed very well about the risk of hand burns due to handling of hot glassware and demonstrated
the use of heat resistant gloves for handling hot glassware.
He demonstrated right placement of hands and body posture was correct.

No such tools were used in this activity however fume hood was utilized and he briefed well, how to use
it properly and why to use it.

Fume hood was checked for proper working condition, by closing and

opening the sash of it.

He discussed when it is necessary to obtain other's help in this activity, as he is aware of the task and
risk behind it.

He discussed the task related risk to hands very well and can brief his peers

as well when required.

He knows when there is a learning from any experience, must be share with the peer to safe
everybody's hands, and also he briefed about his events of stopping peers from various unsafe acts if he
sees something harmful to them.
