AFFIDAVIT OF TWO DISINTERESTED WITNESSES

We, MARINA GABAD and ISMAEL GARAY, both of legal age, Filipinos and residents
of Brgy. Bolo, Matnog, Sorsogon, after having been duly sworn in accordance with law, hereby
depose and say;

1. That we personally know GEMMALINE GEVERA HADAP who was born on

December 9, 1980 at Poblacion, Sta. Magdalena, Sorsogon to spouses
ALFREDO G. HADAP and CONCHITA GEVERA;

2. That we know for a fact that after graduating from high school she had been
employed with Manila Southern Associates, Inc., Robinson’s Incorporated,
Metropolis Management and Staff, Zapote Manpower Corporation and Elite
Garments International I;

3. That unfortunately we recently discovered that her name was erroneously

stated in her Employment Records with said companies as “GEMMALYN G.
HADAP” instead of “GEMMALINE GEVERA HADAP”;

4. That we hereby attest that her true and correct married name is
“GEMMALINE GEVERA HADAP” and not “GEMMALYN G. HADAP”,
the former being the name known to us in the community where we belong;

5. That we hereby attest that the names “GEMMALINE GEVERA HADAP” and
“GEMMALYN G. HADAP”, refers to one and the same persons;

6. That we know these facts because we are close neighbors and acquaintances;

7. That we have no interest in the personal circumstance of GEMMALINE

GEVERA HADAP;

8. That we can attest to the truthfulness of the statements contained herein and for
any purposes this may serve.
