The unit is divided into two parts (1 & 2)

Part 1 (LO1 & LO2):

You are required to produce a word-processed report and the recommended word limit is
2000 words.

Assignment Context and Scenario


Working in any health and/or care setting, every staff member is responsible for
maintaining records and reporting outcomes to supervisors. As a supervisor or manager,
you will likely also have the added responsibility of monitoring the records kept by the
team for accuracy, reliability and validity. It is essential that you are confident discussing
arrangements made to complete records designed for use both internally and with partner
organisations or inspecting bodies, as well as the need to store and retrieve records
effectively, either manually or electronically. You will also need to recognise the correct
procedures when sharing and disposing of records in the setting, following legislation(s)
and guidelines.

You are a manager in a local authority setting providing residential and non-residential
care services to individuals with specific needs. There have been several recent articles
in the news regarding the ‘unnecessary’ death of a patient in a local hospital. The media
reports that there was a catalogue of procedural errors in record-keeping in the hospital
that led to the patient’s death. These included incorrect medicine dosage, missed patient
observations due to inaccuracies in shift records, and an incident with the patient’s family
reporting the patient’s chest pains not being recorded. In addition, the patient’s family was
refused access to their health records, despite the family member requesting the records
being authorised by the patient through a lasting power of attorney. Lastly, the personal
details of the patient were leaked to the media by someone who journalists report worked
at the local hospital. As a result of this, and several other breaches in protocol and
practice, the Secretary of State for Health has ordered the national regulator to conduct a
review of the data management processes and procedures across all health and social
care provision in their remit.

To facilitate this, your Director has tasked you with conducting a Self-Assessment Review
of the reporting and record-keeping requirements and processes in an area of service
provision in your setting, to be used in a series of training sessions to be given to all the
care workers that are new to your organisation. You would be required to write a Review
report on the Learning Outcomes below and ensure that you maintain confidentiality.

Activity

LO1: Describe the legal and regulatory aspects of reporting and record keeping in a care
setting
L02: Explore the internal and external recording requirements in a care setting

For this activity, you will produce an evaluative internal review of the reporting and record-
keeping processes in an area of service provision in your setting. Your Review will be
confidential, and evaluate how effective your work setting’s arrangements and processes
for storing and sharing information are, in terms of efficacy and compliance. You will
include a description of the statutory requirements for reporting and record keeping in
own care setting and the regulatory and inspecting bodies’ requirements for reporting and
record keeping in a care setting. You are also required to describe the process of storing
of records in own care setting and explain the reasons for sharing information within own
setting and with external bodies. Give an accurately illustration of the internal and external
requirements for recording information in own care setting.

For higher grades, you will do an analysis of the implications of non-compliance with legal
and regulatory aspects of reporting and record keeping. Your evaluation will put forward
recommendations for how your setting can improve its processes, with reference to the
consequences of ineffective systems for service user safety, the setting’s effectiveness
and credibility and with reference to the media. Also you should examine the current
processes in own care setting and evaluate the arrangements and processes for storing
and sharing information, making recommendations for improvement.

Part 2 (LO3 & LO4):

You are required to produce a word-processed report, which will also be presented to an
audience in the class. The recommended word limit is 2500 words.

Assignment Context and Scenario


Scenario
With the increasing use of technology in the workplace, an inspirational manager needs to
examine the uses of technology in a healthcare setting and be able to guide their team
through both the procedural use of and the ethical issues surrounding recording and
reporting using technology. This assignment will provide you with the opportunity to not
only demonstrate your knowledge of record-keeping and reporting in a variety of
situations, but also to produce a relevant, accurate set of documents to evidence your
skills.

As a result of your review, your Director has asked that you run one of the training
sessions for new staff. The session will train new staff on the use of technology for
reporting and recording service user care in your setting, providing the staff with a
demonstration of how specific records are completed, processed and stored in your

setting. As part of the training session, you are to provide the staff with a range of
anonymised examples of the records you have kept in the setting, explaining how you
completed, processed and stored these records in line with the setting’s policy, local and
national policies and guidelines.

To demonstrate own understanding of the LO3 & LO4 criteria, you will do an individual
presentation and produce a report.
In the presentation, you will describe how technology is used in recording and reporting
in own care setting. There will also be an explanation of the benefits of involving service
users in record keeping processes. Furthermore, produce accurate, legible, concise and
coherent records regarding service user care for different service users following own
setting’s guidelines. And explain different aspects of own management of service user
records with reference to compliance with national and local policies and guidelines

In the report, you will review the use of digital technology in relation to own medical
management procedures or care plan and evaluate the effectiveness of the use of
technology in terms of meeting service user needs, ensuring appropriate care is given
and maintaining confidentiality. Also, you will analyse the process of maintaining records
in own setting, identifying any potential or actual difficulties. Furthermore, evaluate the
effectiveness of the use of technology in terms of meeting service user needs, as well as
the effectiveness of own completion of documentation.

NOTE: This requires the submission of evidence from a mock training event on record-
keeping. This will include a set of materials used in the event to include an electronic
presentation and you must provide evidence of your slides (10-minute presentation).
Provide evidence from your work on your own record-keeping across a range of types of
records in your own work setting. A range of record(s) could include care plan, minutes of
meetings, recording nutritional status, recording interventions, and administration of
medication, reporting incidents, accident or near misses. (Speak to your tutor for advice
on the type of record you have completed in care practice) Present your evidence in a
presentation (10-minute presentation)

The presentation is compulsory and can only contribute to LO3 & LO4 at the Pass
level.

Presentation Guidelines:
1. Students will have 10 minutes to present their work to the audience and each will
be given the date to present. Presentations will take place in week 11 and week
12 during class time. It is important that all students keep to the time limit.
2. Students must present their work as a PPT only.
3. Students should reference their work where appropriate, using a range of sources.
4. Students must attach their presentation as a (6 SLIDES PER PAGE) document to
their final assignment before submitting on the VLE

Guide to student

1. Preparation guidelines of the Coursework Document


a. All coursework must be word processed.
b. Any assignments submitted with TEXT BOXES will not be accepted.
c. Document margins must not be more than 2.54 cm (1 inch) or less than 1.9cm (3/4
inch).
d. Font size must be within the range of 10 point to 14 points including the headings and
body text (preferred font size is 11).
e. Standard and commonly used type face, such as Arial and Times New Roman, should
be used.
f. All figures, graphs and tables must be numbered.
g. Material taken from external sources must be properly referred and cited within the
text using Harvard system
h. Do not use Wikipedia as a reference.

2. Plagiarism and Collusion


Any act of plagiarism or collusion will be seriously dealt with according to the College
regulations. In this context the definitions and scope of plagiarism and collusion are
presented below:

Plagiarism is presenting somebody else’s work as your own. It includes copying


information directly from the Web or books without referencing the material; submitting
joint coursework as an individual effort.
Collusion is copying another student’s coursework; stealing coursework from another
student and submitting it as your own work.
Suspected plagiarism or collusion will be investigated and if found to have occurred will
be dealt with according to the College procedure (For details on Plagiarism &
Collusion please see the Student Handbook).

3. Submission
a. Initial submission of coursework to the tutors is compulsory in each unit of the course.
b. The student must check their assignments on ICON VLE with plagiarism software
Turnitin to make sure the similarity index for their assignment stays within the College
approved level. A student can check the similarity index of their assignment up to five
times in the Draft Assignment submission point located in the home page of the

c. All Final coursework must be submitted to the Final submission point into the Unit
(not to the Tutor). The student would be allowed to submit only once and that is
the final submission.
d. Any computer files generated such as program code (software), graphic files that form
part of the coursework must be submitted as an attachment to the assignment with
all documentation.
e. Any portfolio for a Unit must be submitted as an attachment in the assignment

4. Good practice
a. Make backup of your work in different media (hard disk, memory stick, etc.) to avoid
distress due to loss or damage of your original copy.

5. Extension and Late Submission


a. If you need an extension for a valid reason, you must request one using an
Exceptional Extenuating Circumstances (EEC) form available from the Examination
Office and ICON VLE. Please note that the tutors do not have the authority to extend
the coursework deadlines and therefore do not ask them to award a coursework
extension. The completed form must be accompanied by evidence such as a
medical certificate in the event of you being sick, and should be submitted to the
Examination Office.
b. Late submission will be accepted and marked according to the College procedure.
It should be noted that late submission may not be graded for Merit and Distinction.
c. All late coursework must be submitted to the Late submission point into the unit
(not to the Tutor) in the ICON VLE. A student is allowed to submit only once and
that is also treated as the final submission.
d. If you fail in the Final or Late submission, you can resubmit in the
Resubmission window.
