การวินิจฉัยและรักษาโรคโลหิตจางจากการขาดธาตุเหล็ก (Iron deficiency anemia)

การวินิจฉัยทางห้ องปฏิบตั ิการประกอบด้ วย 2 (Level of evidence = Level II)
1. การตรวจ CBC และ red cell index จะพบ MCV, MCH มีคา่ ตํ่ากว่าปกติ RDW จะมีคา่ สูงกว่าปกติ (>14.5%)
2. การดู blood smear ซึง่ จะพบ hypochromic microcytic
3.การตรวจยืนยันประกอบด้ วย
1. ค่า serum ferritin < 30 ng/ml หรื อ
2. transferring saturation (TS) < 15%
การคํานวณ TS จาก serum iron (SI) และ total iron binding capacity (TIBC) ได้ ดงั นี ้ TS = (SI/TIBC) X 100
หมายเหตุ ค่า serum ferritin ที่พบว่าสูงกว่าปกติ ต้ องคํานึง ถึงภาวะอื่นเช่น inflammatory disease หรื อ cirrhosis
เพราะค่านี ้เป็ น acute-phase reactant จะสูงขึ ้นได้ ในภาวะดังกล่าว
