Oliguria
Definition
Infants : urine output < 1 mL/kg/h
Children : urine output < 0.5 mL/kg/h
Adults : urine output < 400 mL/d
22
Definition of Acute Renal Failure
• Acute renal failure is an abrupt decline in
glomerular and tubular function, resulting in the
failure of the kidneys to excrete nitrogenous
waste products and to maintain fluid and
electrolyte homeostasis.
• Azotemia is a consistent feature of acute renal
failure, oliguria is not.
23
Causes of Acute Renal Failure
Prerenal
~55%
Intrarenal/Itrinsic ~40%
Postrenal ~5%
24
Acute renal failure
• Prerenal causes
– Shock/hypovolemic e.g. hemorrhage, dehydrate
– Apparent intravascular hypovolemia e.g. CHF
– Vascular e.g. renal artery occlusion
• Renal causes
– Acute tubular necrosis e.g. ischemia, nephrotoxic drugs
– Acute interstitial nephritis e.g. nephrotoxic drugs,
infections
– Acute glomerular diseases e.g. malignant
nephrosclerosis , acute glomerulonephritis,
Goodpasture ‘s syndrome
– Chronic renal failure
• Postrenal causes
– Urethral obstruction e.g. BPH, urolithiasis
– Ureter obstruction e.g. tumor
– Intratubular obstruction e.g. crystals from medication 25
(acyclovir, sulfonamide and methotrexate)
Suspected disease
Renal causes
- Acute tubular necrosis e.g.
Prerenal ischemia, nephrotoxins
causes exposure
- Vascular
- Acute interstitial nephritis e.g.
e.g. renal
nephrotoxic drugs
artery
occlusion - Acute glomerular diseases
e.g. malignant nephrosclerosis
26
ATN vs AIN
Acute tubular necrosis Acute interstitial nephritis
-Acute renal failure due to tubular -Interstitial edema and tubular cell
damage damage
-The two major causes are ischemia -Cause by drug, infectious diseases,
and nephrotoxin exposure immunologic disorders, or idiopathic co
ndition
-Hyperkalemia and hyperphosphatemia -Fever, transient maculopapular rash,
arthralgias, peripheral blood
eosinophilia
-Urine may be brown with pigmented -The urine often contains red blood
granular casts, renal tubular epithelial cells ,white blood cells, and white blood
cells and epithelial casts cell casts.

Summary
41
– PGC = Hydrostatic pressure in glomerular
capillary
– ¶GC = Colloidal osmotic pressure in glomerular
capillary
– PBC = Hydrostatic pressure in Bowman’s space
– ¶ BC = Colloidal osmotic pressure in Bowman’s
space
Regulation of GFR & Renal blood flow
44
Renin Angiotensin Aldosterone System
arteriole )

Physiologic control of GFR and Renal
blood flow
1. Sympathetic NS Hormone or Effect on GFR
autocoid
activation: all blood
Norepinephrine ↓
vessels are richly
innervated. Epinephrine ↓
Most important during
severe acute Endothelin ↓
disturbances
Angiotensin II ↔ (prevents ↓)

Use of clearance method to
quantify kidney function
• The rates at which different substance are
cleared from plasma provide a useful way
of quantifying the effectiveness of which
the kidney excrete various substances
Cs = Us x V / Ps
• Cs = clearance rate of a substance s
• Ps = plasma concentration of the substance
• V = urine flow rate
• Us = urine concentration of the substance

CHEMICAL BUFFERING MECHANISM
• HCO3-/H2CO3
• HPO42/H2PO4-
• Hb
• PROTEIN
CO2& carbonic anhydrase) 53
