5.18 Health, safety and environment (HSE)
Hazards associated with slips, trips and falls were specified in the Safety Management Manual, describing “PPE Requirement,
Manual Handling, Means Of Access and Safe Access to Deck”. Deck walkways were painted with anti-slip paint. Ship’s
bulwarks, railings and gratings were noted in a good condition, whilst the decks including the engine room were free of oil or wet
that could cause slips.
Obs.3: Forward working deck was not provided with non-slip walkway
14.13 Bridge, navigation and communications equipment
The vessel was equipped with a GMDSS radio installation. Instructions for operating the Digital Selective Calling
(DSC) and satellite communications equipment in an emergency were prominently displayed on the bridge, supplemented by
the vessel’s call-sign and ship station identity. A continuous listening was maintained on VHF channel 16. Periodical test of radio
equipment was regularly conducted. The Master, Chief Officer and Second Officer were noted holding a valid GMDSS (GOC)
Certificate.

Obs.4: SSB radio was noted as under maintenance.


15.6 Machinery space
The vessel was equipment with emergency generator which tested in good working order, well maintained & regular test was
reviewed.

Obs.5: High pressure fuel delivery pipe joins for emergency generator were not wrapped with “anti-splash” tape.

7 Ship and cyber security
The unit is running at ISPS code is noted Mandatory.
11 Firefighting appliances
Sufficient Fire Fighting Equipment’s were provided on board and noted accordance with the vessel’s type andoperation. Through
random check and review, the crewmembers were apparently to be familiar with operation procedures and location.
12 Pollution prevention
Safety drill for prevent oil pollution at sea was conducted regular period (three monthly) and crews were apparently to be familiar
for prevention of oil pollution. Vessel has sufficient equipment’s for oil spill prevention was available on board whilst over board’s
valve in Engine room had been locked and warning notice posted.
14 Bridge, navigation and communications equipment
Vessel has sufficient radio equipment accordance with vessel’s type and operation. Reportedly, Maintenance programming for
Radio, Navigation and Electronics equipment were noted to be carried out by shore competent technician.
15 Machinery space
Engine room was noted 24 hours manned whilst machinery; Diesel generators, pumps, electrical and alarm system were noted
working properly whilst Engine room cleanliness was noted maintained.
16 Mooring, towing and lifting equipment
In general mooring arrangement was sufficient for the vessel’s typical operation.
S1 Survey vessels (including offshore seismic survey)
The unit was Seismic Vessel and Seismic equipment visually noted in satisfactory condition

Debrief

The inspector shall discuss the inspection findings with the master before leaving the vessel. The close-out meeting
report should be completed (template available in the inspection application under File > Download documents)
and submitted with the final report as an attached image file.

Distribution list for reports

A written copy summarising the findings should be left on the vessel inspected.
