Sir Kent, what brings you here?
Mr. Kent stated that he’s experiencing back pain

C. History of Present Illness 

When did you experience this pain?
Mr. Kent have been experiencing this pain for over a week now
Can you describe how severe the pain is?
Mr. Kent said that the location of the pain is in the lower right part of his back and it seems like
the pain is going up.
Were you doing something before you experience this pain?
Mr. Kent said that he doesn’t remember doing anything that may trigger what he’s experiencing.
Did this happened before?
Mr. Kent said this is the first time.

D. Past Medical History 

When did you last went to a hospital to be consulted?
Mr. Kent stated that last time he went to a hospital was when he was confined because of typhoid
fever and that was 2 years ago.
Did something happened before? Like accidents that may triggered what you’re feeling right
now?
He said he doesn’t remember anything.

E. History of Family illness

Do you still live with your parents?
Mr Kent stated that he does.
Do you have member/s in your family who experienced the same thing as what your
experiencing right now?
Mr. Kent said that his father once experienced the same thing, his father had to go to a physical
therapist to treat his back pain.
At what age did your father started experiencing the symptoms?
Mr. Kent said that his father started experiencing the symptoms on his early 40s.

Mr. Kent stated that when he’s reviewing lectures and his back started to hurt, he will rest for
about 15 mins

Do you have any religious beliefs that helped you with your health?
Mr. Kent said that there are days that he just wanted to breakdown but instead or crying, he
prays. 

Mr. Kent is experiencing from a back pain, specifically on the lower right part. He’s in pain for
already a week now. He also said that he doesn’t eat and sleep properly but his urine and bowel
output are said to be normal. There is a possibility that he inherited his back pain to his father.
His condition made him think that he is less of a person. However, it doesn’t affect his mood,
decisions in life as well as his relationship with other people.
