Please complete this form digitally within the first week of starting your placement and email
it back as a Word document file to placements@herts.ac.uk.

The following items should be included in your induction into the organisation, preferably within the first
week. Please check off the items below when they occur and inform your placement organiser of any
items not covered within one week of the start of your placement. This list is not exhaustive and other
topics may be covered, which you may note if you wish:

Please be aware that your employer has a responsibility to check your eligibility to work in the UK. You
should have a right to work check carried out by your employer at the start of your employment. Any
issues that arise from the right to work check may mean that your employer ceases your employment
with immediate effect.
