SUBJECT : REQUEST FOR THE FIRST (1ST) QUARTER CASH ADVANCE

TO COVER MOOE FOR THE NHA PROJECT
IMPLEMENTATION TEAM (NPIT-B) REGION VI (ILOILO,
CAPIZ, AKLAN, & ANTIQUE) FOR THE YEAR 2021.

May we request for the release of First (1 st) Tranche to set-up for the First (1st)
Quarter Budget to cover the Maintenance and Other Operation Expenses for NHA
FAO NPIT-B, Region VI (Typhoon Yolanda) MOOE in the amount of TWO
MILLION NINE HUNDRED THIRTY THREE THOUSAND TWO HUNDRED PESOS
ONLY (P 2,933,200.00).

The above-mentioned amount comprises the projected three-month expenditure of

NPIT-B Region VI (Iloilo, Capiz, Aklan, & Antique) covering specifically the months of
January to March 2021.

The fund shall be deposited in the same authorized Land Bank of the Philippines
branch where the existing revolving fund account is maintained, to wit;

Subject fund shall be drawn on a quarterly basis through a cash advance amounting
to Two Million Nine Hundred Thirty-three Thousand Two Hundred Pesos Only (Php
2,933,200.00). Please consider however, that for the first quarter and onwards of
year 2021, we are recommending that said funds be released under the name of Ms.
Fe T. Jamoyot, being the OIC-District Manager of Iloilo/Guimaras District Office. As
such we are also requesting an office order designating Ms. Jamoyot as the
Disbursing Officer of the said funds for the four-quarter budget of year 2021.

The fund shall be deposited at the existing MOOE depository bank account of NHA
Project Implementation Team-B (NPIT-B), Region VI (Iloilo, Capiz, Aklan and
Antique) under LBP CA No. 3292-1044-47, “NHA FAO NPIT-B, Region VI Office
(Typhoon Yolanda) MOOE” with the following officers/staff of Region VI Office as the
authorized signatories for the disbursement from the fund:

May we request an office order designating Ms. Fe T. Jamoyot, OIC-District

Manager, Iloilo/Guimaras District Office, Region VI as the new Disbursing Officer of
the MOOE fund of NPIT-B for the Year 2021.

The Four (4) quarter fund requirement of NHA Project Implementation Team B
(NPIT-B) Region VI covering the provinces of Iloilo, Capiz, Aklan and Antique for
year 2021 is broken down as follows:
