Subject: DEMAND TO PAYMENT AND VACATE

Dear Mr Vid:

This letter is a formal notification that the complainant, Ms. Luck Dawn, is asking
you, Mr. Con Vid, to permanently vacate the premises of 125 Ludwig Street, Forbes
Park, Lucena City, Quezon. This is also to inform you that you have failed to pay Nine
Thousand (9,000.00) rent for three months, October, November and December, which
is a violation of the lease agreement, and an additional payment of Fifteen Thousand
(PHP15,000.00) for the unpaid rentals form January 2020 to May 2020. This is to give
you a 30-day grace period or until May 20, 2020, to pay the rental in full or you would be
ejected.

If you remain in the premises on or after the date that you are specified to vacate,
then the termination will be enforced through the judicial process. At this time, you can
present a defense. If a meeting is requested, then I will discuss to you further your
violations and the proposed leased termination.
