Initial Incident Report – Collapse of Crane at

Hindustan Shipyard Limited (Vizag)

Date: - 01.08.2020 (Between 11.00 AM & 12.00 Noon)

10 workers crushed to death after crane collapses in


Visakhapatnam shipyard (The massive crane suddenly crumbled
and crashed to the ground with a massive sound)
1- The crane was a newly erected and was undergoing trial runs to
ascertain its capacity & Inspection when it collapsed between 11
a.m. and noon.
2- Approx. 20 employees were involved in the trial run, load testing &
Inspection
3- Crane used to carry equipment for shipbuilding.
4- The new crane was bought from Anupam Crane Company in
Mumbai as part of its upgradation of cranes in HSL. But Anupam
Crane Company did not conduct the trail run during delivery.
5- Full load Testing was going on by the company engineers & local
contractors.
6- Crane was not handed over to HSL by M/s Anupam Cranes.
7- Anupam Company had supplied a crane to the HSL at a cost of Rs
50 crore five years ago. But it has not conducted the trail Runs.

Probable Cause (Trade Union Statement): -

• Crane was over loaded while carrying out the load testing
• Employees were not competent enough to carry out the load testing &
inspection
• Testing was being carried out without Safety measures

Preventive Measure: -

• Trail runs and load testing should be carried out by the competent person
/ OEMs
