Findings:-The ground surface where the hydra was being placed to lift biltop was not flat
and physically we observed that it was more than 20-degree slope which cause hydra to
became unstable with suspended load in fact it was tilled at the time of placement .
The direction of hydra at the time of operation was not right; the operator lifted the biltop
by placing the hydra in parallel position rather than perpendicular.
The reason behind the cause was un-safe act as we found that the hydra placed for
operation was not safe due to slope however enough flat surface was available around the
fabricated biltop and the operator could have placed the hydra at uniform surface.
The another reason behind the cause was behind of the tire pressure was very low.
At the time of operation the boom length was not extended. As per SWL the weight of
the biltop was quite low.
