the same time from RD Massage Salon. Both parties discuss the
issue on the under time of the OJT Trainee Mae Joy Muralla. The
resolution of the problem is they both agreed that the trainee should
cover up the lacking number of hours.

MEMORANDUM OF AGREEMENT

KNOW ALL MEN BY THESE PRESENTS:


This Memorandum of Agreement is made and executed by and among:

RD Massage Salon a non-stock, non-profit foundation engaged in


technical vocational training duly registered with the Securities and
Exchange Commission with school address located at BRGY. CAMPETIC
PALO, Leyte, Philippines represented in this act by its Branch Manager,
Rose Anne Patrimonio and hereafter referred to as the Company;
-and-
TESDA, a duly accredited educational institution with business
address at Abucay, Tacloban, City herein represented by the Training
centers Administrator, CHANTAL HIPE, herein after referred to as the
SCHOOL.

-WITNESSETH-

WHEREAS, the SCHOOL, an accredited institution of learning that


offers MASSAGE THERAPY NC II, seeks to provide its students with
necessary exposure and training in actual worksites to prepare them for
employment upon graduation.
WHEREAS, upon request of the SCHOOL, the COMPANY has agreed
to admit student-trainees to the COMPANYS’ on-the-job-training (OJT)
program in support of the COMPANYS’ objective of providing students with
actual on-the-job experience to match the academic curricula with the
industry demands.
NOW, THEREFORE, in consideration of the foregoing premises, the
parties hereby agree as follows:

1. The SCHOOL considering the requirements set by the COMPANY,


shall pre-screen and endorse to the COMPANY a list of students-
trainees for OJT placement.

2. The SCHOOL shall invite the COMPANY to participate in its job-


placement activities.

3. The COMPANY shall require each student-trainee to submit a Letter of


Recommendation from the SCHOOL before admission to the OJT
Program.

4. The OJT shall be held within the COMPANY premises and scheduled
on April 01, 2020 and will end on June 18, 2020 during the hours
8:00 AM to 5:00 P.M. Monday to Friday only.

5. The duration of the OJT Program shall be four hundred twenty four
(424) subject to the item no.4 above.

6. The COMPANY through the Practicum Supervisor agreed to


accomplish the following:

a. Provide student-trainee with appropriate task that will give them


ample and practical work experiences in their field of
specialization.
b. Assist the student-trainee in the preparation of the work outline
sheet;
c. To regularly monitor and evaluate the performance of the student-
trainee during entire internship period.

IN WITNESS WHEREOF, the parties have signed this Memorandum


of Agreement this March 23, 2020 at RD Massage Salon, Palo, Leyte
