1. Explain with your own words what is Nurse Care Plan.
2. Explain what are the functions of Nurse Care Plan.
3. Mention and Explaining the constraints that you were found in your daily work when
you were making nurse care plan for the patients in your hospital.
Answer :
1. Nurse Care Plans is planning is a part as the organizing phase the nursing process as
guid to direct nursing action in an effort to help, alleviate, solue problem,and to meet
patient need.
2. The function of Nurse Care Plase is a
a. Administrative objectives
- To identify the focus of nursing on the patient or group
- To distinguish the responsibilities of nurses and other health professions
- To provide a criterion for nursing report and evaluation
- To provide patient clasification critteria
b. Clinical Goal
- Provides a writing guidelive
- Communicate nursing staff what is done
- Provide outcome criteria as repeat and evaluation of nursing
- Direct specific plan of action for individuals, families, and other healt
proffessionals to carry out the action
3. The constraints that were found in daily work
- In assesing is out of the design factor in determining nursing plans, due to the lack
of knowledge of nurses in assessing patient, how to make nursing care plans can
not be implemented optimally.
- Heavy workload and the number of task gruen affect nurses in making nursing care
plans less than the maximum and the number of nurses that are not in accordance
with the number of patients treated and the level of patient dependence.
- Implementation of the nursing care documentation in phase is part the not arrage in
ovder of priority and the nursing action plans lacks patients and family
involvement.

CASE:
A 35 years old female said that she had a chest pain radiating to the left arm, back and neck.
She always had fatigue, weak pulse, shortness of breath and pain scale of 9 to 10.
